RE: APPROVAL OF AGREEMENT BETWEEN THE WELD COUNTY AMBULANCE SER-
VICE AND THE WELD COUNTY GENERAL HOSPITAL AND AUTHORIZATION
FOR CHAIRMAN TO SIGN THE SAME.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Weld County Ambulance Service is interested
in educational opportunities that may be available at the Weld
County General Hospital and desires to utilize standby time
of ambulance attendants at said hospital , and
WHEREAS, an Agreement between the Weld County Ambulance
Service and the Weld County General Hospital has been prepared
and submitted to the Board of County Commissioners for approval,
and
WHEREAS , the Board, after studying said Agreement, deems
it advisable to approve the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the Agreement between
the Weld County Ambulance Service and the Weld County General
Hospital be, and hereby is , approved.
BE IT FURTHER RESOLVED by the Board of County Commissioners
that the Chairman of the Board be, and hereby is , authorized to
sign said Agreement.
The above and foregoing Resolution was , on motion duly made
and seconded, adopted by the following vote on the 9th day of
February, A.D. , 1981.

AGREEMENT
THIS AGREEMENT is made and entered into this 21st day of January
1981 , by and between the WELD COUNTY AMBULANCE SERVICE, a department of Weld
County, Colorado, hereinafter referred to as "AMBULANCE SERVICE" and WELD COUNTY
GENERAL HOSPITAL, hereinafter referred to as "HOSPITAL" .
WITNESSETH:
WHEREAS, Weld County Ambulance Services desires to utilize standby time of
ambulance attendants in the medical institution, and,
WHEREAS, the Regional Health Plan has recommended ambulance service by
dispatched from hospitals, and,
WHEREAS, Weld County General Hospital is interested in cooperating with
Weld County Ambulance Service to make provisions for dispatching ambulances
from the Hospital as a means to assist in providing high quality ambulance
services to the area, and,
WHEREAS, the Ambulance Service is interested in educational opportunities
that may be available in the Hospital ,
NOW, THEREFORE, for and in consideration of the promises and mutual agree-
ments contained herein, the Ambulance Service and the Hospital enter into the
following conditions :
The Ambulance Service agrees :
1 . To provide two persons assigned to the Hospital for work assignments
on the 7:00 a.m. to 3:30 p.m. shift and two persons on the 3:00 p.m. to 11 :30 p.m.
shift and two persons on the 11 :00 p.m. to 7:30 a.m. shift seven days per week.
2. To designate one person in charge of ambulance personnel for each shift.
Hospital personnel will report daily work requests for ambulance personnel to the
ambulance in charge person.
3. To provide paging and communication systems and equipment for its
employees when requested by the Hospital .
4. To maintain adequate Workmen's Compensation, malpractice and liability
insuranc e to cover its employees while engaged in Hospital activities and to
furnish the Hospital with certification of such insurance.
5. To comply with all rules, regulations and directives of the Hospital .

6 . To perform only those duties and activities assigned to ambulance
personnel by the Head Nurse of the Emergency Department in accordance with
Hospital Policies, Rules, and Regulations, Protocols, Directives, and Pro-
cedures. Duites assigned are included in the attached Addendum.
7 . Agrees to indemnify and hold harmless the Hospital from any liability
caused by any employee of Weld County Ambulance Service including any amounts
expended by Weld County General Hospital for attorney's fees or court costs .
8 . To pay the Hospital $1120.00 each month for the rent of office and
garage space until May, 1981 . After May, 1981 ,$350 will be paid each month
for the rent of garage space. The amount will be deducted each month on the
bill submitted by the Ambulance service to the Hospital .
The Hospital agrees:
1 . To pay the Ambulance Service $4152 each month for two (2) employees
per shift (except in emergency situations) , excluding time for lunch periods
and time for Ambulance Service activities including all educational and train-
ing activities. Ambulance personnel will also check out for other ambulance
activities such as training, maintenance activities, and all other activities
related to ambulance business.
2. To provide parking facilities for ambulance vehicles on call .
3. Priority of attendants is to ambulance responsibilities .
Both parties agree:
That the Hospital will not be responsible for the full salary of ambulance
personnel for the duty assignments will have to provide for interruption and
quick departure and the first responsiblity of ambulance personnel will be to
fulfill the obligations of the Ambulance Service.
That the Hospital may refuse the services of any individual ambulance atten-
dant.
That this Agreement may be amended at any time by written endorsement hereto,
agreed to and signed by both parties.
That this Agreement shall be for the period of January 1 , 1981 through Decem-
ber 31 , 1981 .
That this Agreement shall be subject to cancellation by either party giving
the other party written notice sixty (60) days prior to the cancellation date.

That it is expressly understood that Weld County Ambulance Service is an in-
dependent contractor and shall be solely responsible for its actions and those of
its employees.

This contract is not assignable without the written consent of Weld County
General Hospital .

AGREEMENT ADDENDUM
DUTIES AND ACTIVITIES ASSIGNED TO
WELD COUNTY AMBULANCE PERSONNEL AT WELD COUNTY GENERAL HOSPITAL
PURPOSE:
To develop and improve the skills and knowledge of the Ambulance personnel
by providing the clinical exposure and continued education obtained through the
various aspects of Emergency Patient Care. To function as an active member of
the nursing staff providing direct care for the safety and comfort of the patient.
1 . Weld County Ambulance personnel are primarily and directly responsible to
the Weld County Ambulance Service.
2. In the Emergency Department, the Ambulance personnel receive daily work as-
signments from the Head Nurse, physician or Nursing personnel on duty.
3. Ambulance personnel are encouraged to attend the regular Emergency Department
meetings as well as all the available inservices related to Emergency Care,
when possible.
Personnel who have successfully completed the EMT and Paramedic Course and
have the ability and training as verified by the Director of the Ambulance Com-
pany may perform the following skills under the direction and supervision of the
nurse and/or physician. However, it is understood that ambulance calls take pri-
ority over the following:
1 . Takes and records vital signs and brief medical history on assigned patients .
2. Gives verbal reports to nurses and/or physicians, on assigned patients.
3. Completes hospital diagnostic requests.
4. Completes admission information on Emergency Department patients in the absence
of the clerk.
5. Registers patients in the log book in the absence of the ward clerk.
6. Performs all duties of the orderly in his absence.
7. Answers Emergency Department phones and refers calls to appropriate physician
or nurse.
8. Is familiar with the location of and able to utilize appropriate Emergency De-
partment equipment, including the taking of EKG's when requested and needed by
Hospital personnel .
9. Assists other Emergency Department personnel with stocking and supplies of the
Department.

10. Assists other Emergency Department personnel with cleaning of instruments
and securing supplies from Central Supply and in general clean-up of area.
11 . Responds to in-house or Emergency Department code arrests and handles car-
diac massage or ventilation as requested by the code leader.
12. Recognizes common fractures and utilizes proper techniques in splinting.
13. Cares for all open or closed wounds and hemorrhage.
14. Recognizes signs and symptoms of medical emergencies and the complications
due to trauma, e.g. shock, respiratory distress, MI, etc. May institute
life support measures until physician control can be established.
15. Cleans and irrigates lacerations and assists the physician with suturing.
16. Assists with initial treatment of burn patients.
17. Performs eye irrigations.
18. Catheterizes patients.
19. May start IV fluids and/or draw venous samples.
20. Paramedics may administer drugs by the oral , IM, sub-Q and IV tubing route.
21 . Assists with application and removal of casts.
22. Operates the Poisindex for poison control and refers all poison calls to nurse
or physician.
23. When bringing in patients from the field, the Ambulance personnel who attended
the patient will remain with the patient until released by the physician or nurse.
24. If the Emergency Department team of Ambulance personnel is called to the field,
the 2nd, or office crew, will contact the Emergency Department to see if and how
soon they should report to the Emergency Department.
25. One Ambulance personnel and one Nursing personnel will work with a critical
patient in the Emergency Room leaving other staff to care for the rest of the
patients. Their responsibilities shall continue until the patient is delievered
to CCU/ICU.
