Caring for Colorado is a nonpartisan, statewide health foundation that focuses on strengthening community health and supporting programs that reduce unintended pregnancies and improve family well-being. Through its Choose When 2026 program, it provides targeted support to reduce financial barriers to contraception services. What options exist for the Board? • Approve the request • Deny the request Consequences: If approved: PHS can request $50,000 to improve contraceptive access, reduce financial barriers, and support same-day LARC availability for Weld County residents. The power to decide if and when to start a family is essential for an individual's health, autonomy, and future. Choose When is a program of Caring for Colorado working to bring down financial barriers to contraception for people who are uninsured, underinsured, or face other obstacles to accessing the contraceptive method of their choice. Funded by private philanthropy and individual donors, Choose When provides financial support to safety net clinics across Colorado to cover the costs of contraception, especially long-acting methods,to improve access to this basic healthcare need. OPEN TO:Colorado-based safety net health clinics, public health agencies,school-based health centers, and rural health clinics that fit the eligibility criteria. GRANT AMOUNT: Up to$50,000 for a 12-month period.The requested amount should reflect the number of patients of reproductive age served by your clinic who require financial support for contraception (uninsured, underinsured, or who require confidential services). WHAT WE WILL FUND: • The purchase and stocking of all methods of contraception to ensure that people with financial barriers to care can access the method of their choice, ideally on the same day that they access healthcare. Choose When will also cover the costs of vasectomy care as a contraceptive method. • The cost of office visits to provide person-centered contraceptive care. • Health educators who work in partnership with healthcare providers on one-on-one or small group education on reproductive and sexual health that is inclusive of cultural or 2SLGBTQIA+ needs. • Provider training to ensure that people can access the contraceptive method of their choice. WHAT WE WILL NOT FUND • General operating support of the clinic. • Lobbying or political support. • Technical projects,such as electronic health records, billing, and coding improvements. GRANT EXPECTATIONS:Choose When funds are only available to increase access to contraceptive care for patients without a payer source or adequate health insurance,or who require confidential services.The goal is for patients to receive the contraceptive method of their choice, ideally with same-day service,free of charge, or at a highly discounted rate. Grantees are expected to collect basic data related to how the grant funds are spent, including: • Breakdown of allowable expenditures (i.e., purchase of contraceptive methods,cost of office visits to provide contraceptive care, health education on sexual and reproductive health, provider training,and focused outreach) • The number of unduplicated patients who benefited from Choose When funding and any reportable aggregate information about insurance status, age,or self-identified race, ethnicity, or gender identity. Choose When will work with grantees to ensure that any data collection activities uphold all confidentiality standards and requirements. • Other evaluation activities may include interviews with the clinic director or staff who managed the grant. Grant recipients may be asked to participate in an external evaluation of the Choose When Program. Caring for Colorado is requesting this information from grantees to understand the landscape of contraceptive access in Colorado and identify system and policy changes that are needed to reduce financial barriers to contraceptive care. Weld County Department of Public Health and Environment(WCDPHE) provides comprehensive reproductive health services through its Title X family planning clinic,the only Title X provider serving Weld County residents. Person-centered contraceptive care is a core component of our clinical services and aligns with our mission to promote public health and improve access to preventive care. Our clinic provides counseling on the full range of contraceptive options and supports patients in selecting the method that best aligns with their health needs, preferences, and reproductive goals. Weld County has a population of more than 350,000 residents, including tens of thousands of individuals of reproductive age. Patients access contraceptive care through scheduled clinic appointments, same-day visits when available, and referrals from community partners.WCDPHE also conducts outreach throughout the county, including rural communities,to connect residents with services such as family planning, STI testing, immunizations, and other preventive care. Many patients served by WCDPHE face significant barriers when trying to access their preferred contraceptive method.These barriers include lack of health insurance, high out-of-pocket costs for long-acting contraceptive methods, transportation challenges across a large and geographically dispersed county, and limited access to confidential services for adolescents and young adults. Long- acting reversible contraceptives (LARCs), such as intrauterine devices and contraceptive implants, are among the most effective methods available, but the upfront cost of these devices can create financial barriers for patients and limit the ability of safety-net clinics to maintain adequate inventory. WCDPHE works to reduce these barriers by offering services on a sliding fee scale, prioritizing same-day access to contraception whenever possible, and maintaining partnerships with community organizations that help connect underserved residents to care. However,funding limitations can make it difficult to consistently stock higher-cost contraceptive methods such as LARCs.Additional resources would allow the clinic to maintain adequate contraceptive inventory and expand access to highly effective contraceptive options for uninsured and underinsured patients, particularly those living in rural areas or facing financial barriers to care. 2. Describe your organization's patient payer mix and what it reveals about access to contraceptive care in your community. Over the past 12 months (calendar year 2025),Weld County Public Health's family planning clinic provided approximately 1,500 reproductive health visits.The payer mix for these visits reflects the clinic's role as a safety-net provider for residents with limited access to affordable healthcare. Approximately 65%of visits were covered by Medicaid or CHP+,about 30%were for uninsured patients receiving services through the Title X sliding fee scale, and approximately 5%were covered by private insurance. Medicare coverage is rare in this program because most services are provided to individuals of reproductive age. This payer mix demonstrates that a large share of patients seeking contraceptive care in Weld County face financial barriers to accessing services in traditional healthcare settings. Many patients rely on Medicaid coverage or have no insurance coverage at all. For uninsured or underinsured individuals,the cost of highly effective contraceptive methods—particularly long-acting reversible contraceptives (LARCs)—can be prohibitive without additional financial support. As the only Title X provider in Weld County,WCDPHE plays a critical role in ensuring access to affordable contraceptive care for residents who might otherwise delay or forego services.The payer mix highlights the continued need for programs that reduce financial barriers and allow safety-net clinics to provide the full range of contraceptive methods regardless of a patient's ability to pay.
