Approve Lease Agreement for Building Space (914 11th Avenue, Greeley) and
Authorize Chair Pro-Tem to Sign — High Plains Medical, LLC
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with a Lease Agreement for Building Space
(914 11th Avenue, Greeley) between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Facilities
Department, and High Plains Medical, LLC, commencing upon full execution of
signatures, and ending March 31, 2026, with further terms and conditions being as stated
in said lease agreement, and
Whereas, after review, the Board deems it advisable to approve said lease agreement,
a copy of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Lease Agreement for Building Space (914 11th Avenue, Greeley)
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Facilities Department, and High Plains
Medical, LLC, be, and hereby is, approved.
Be it further resolved by the Board that the Chair Pro-Tem be, and hereby is, authorized
to sign said lease agreement.
The Board of County Commissioners of Weld County, Colorado, approved the above
and foregoing Resolution, on motion duly made and seconded, by the following vote on
the 25th day of February, A.D., 2026:

LEASE
THIS LEASE is made and entered into this 5tiday of February, 2026, by and between
the County of Weld, a body corporate and politic of the State of Colorado, by and through its
Board of County Commissioners, hereinafter referred to as "County," and High Plains Medical,
LLC, hereinafter referred to as "Tenant."
WHEREAS, on February 26, 2026, County purchased from Tenant property located at
914 11th Avenue in Greeley, Colorado 80631 (the "Property"), and
WHEREAS,the Property consists of a brick-and-mortar one-story building, and
WHEREAS, Tenant desires to lease and occupy the Property, pursuant to the terms and
conditions contained herein, to and until March 31, 2026, at 11:59 p.m. MDT.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained
herein, the parties hereto agree as follows:
1. TERM. The term of this Lease shall be from February 26, 2026, to and until March 31,
2026, at 11:59 p.m. MDT.
2. RENT. Rent shall be $1.00 for the entire term. County confirms receipt of said rent.
3. POSSESSION. Tenant may possess the Property from February 26, 2026,to and until
March 31, 2026, at 11:59 p.m. MDT. Tenant is free to take any appliances or fixtures
from the Property at the end of this Lease.
4. UTILITIES. During the term of this Lease, water, sewer, electricity and gas utilities
used by Tenant shall be paid by County. Any other utilities used by Tenant during the
term of this Lease shall be paid by Tenant.
5. JANITORIAL AND MAINTENANCE SERVICES. During the term of this Lease,
Tenant will provide its own janitorial and maintenance services. Trash removal shall be
arranged for and paid by Tenant.
6. INSURANCE. Tenant shall, at its own expense, procure and maintain during the term of
this Lease sufficient contents insurance to insure itself against loss of its personal
property.
7. ASSIGNMENT AND SUBLETTING. Tenant shall not assign, mortgage, pledge or
encumber this Lease, or permit all or any part of the Property to be subleased to another.

8. CASUALTY AND UNTENANTABILITY. If the Building is made substantially
untenantable, or if Tenant's use and occupancy of the Leased Premises are substantially
interfered with, by the Property being made wholly or partially untenantable by fire or
other casualty, this Lease shall terminate and no compensation shall be due and payable
by County to Tenant.
9. ENTIRE AGREEMENT. This Lease sets forth all the covenants, promises,
agreements, representations, conditions, statements and understandings between the
County and Tenant concerning the lease of the Property by Tenant, and there are no
representations, either oral or written between the parties other than those in this Lease.
Without limiting the foregoing, Tenant, hereby, specifically waives any claims, rights, or
defenses based on any warranties, representations or guarantees, whatever their form,
made at any time, by any party, negligently made or otherwise, except those warranties,
representations or guarantees contained in this Lease. This Lease shall not be amended
or modified except in a writing signed by both parties. Failure to exercise any right in
one or more instance shall not be construed as a waiver of the right to strict performance
or as an amendment to or modification of this Lease.
IN WITNESS WHEREOF, Landlord and Tenant have executed this Lease as of the day
and year first above written.
