HomeMy WebLinkAbout20260389 Resolution Approve Amendment #4 to Task Order Contract for Syphilis Field Delivered Therapy (FDT) and Jail-Based Sexually Transmitted Infection (STI) Programs, and Authorize Chair Pro-Tem to Sign Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and Whereas, the Board has been presented with Amendment #4 to the Task Order Contract for the Syphilis Field Delivered Therapy (FDT) Program and the Jail-Based Sexually Transmitted Infection (STI) Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing upon full execution of signatures, and ending June 30, 2026, with further terms and conditions being as stated in said amendment, and Whereas, after review, the Board deems it advisable to approve said amendment, a copy of which is attached hereto and incorporated herein by reference. The requested changes align the contract with current program operations by adding activities that are now being implemented and removing activities that are no longer applicable. What options exist for the Board? 1. Approve the requested contract amendment to allow Weld County to continue operating the STI/FDT program in compliance with updated CDPHE requirements. 2. Decline the amendment, which would place the County out of compliance with the current grant agreement and jeopardize continued funding. Consequences: Approval of the amendment allows continued implementation of STI prevention, testing, and treatment services, including field-delivered therapy and jail-based and community outreach activities. Impacts: This amendment does not expand the scope of the program beyond its original public health purpose. It clarifies and formalizes activities already being conducted. Costs (Current Fiscal Year / Ongoing or Subsequent Fiscal Years): There is no change to the total contract amount. No County General Fund impact. No new FTEs are requested; existing staff will support this work. Recommendation: I recommend approval to place this item on a future BOCC agenda for formal consideration. Entity Name: Weld County Department of Public Health("WCDPHE") II. Project Description: This health project serves to prevent new cases of syphilis by screening, identifying,and treating syphilis in persons of reproductive age in Weld County and surrounding counties in NE Colorado (see service area below). Weld County Department of Public Health will provide field-delivered therapy to any individual who is otherwise unable to access treatment in a clinical setting due to barriers. These barriers include, but are not limited to being unhoused, lacking primary or prenatal care, lacking adequate insurance coverage, transportation barriers, or substance use disorders. Additionally, this project seeks to increase early identification of syphilis, access to treatment and referrals, linkage to prenatal care services, which will reduce the risk and burden of syphilis transmission. This project is being funded to address the increasing rates of syphilis in Colorado. HI. Definitions: 1. Advertise-To advertise means a read,written,visual,oral,watched,or heard bid for,or call to attention. Advertisements can be considered communications or marketing based on the intent and content of the message. 2. Active Referrals-Active referrals are providing assistance to a client(s)with accessing the service they are referred to,including but not limited to:assisting clients in making an appointment, completing a Release of Information,providing resources for client to access services,providing appointment reminders,following up with the agency providing the service to confirm client's attendance 3. CDC—Centers for Disease Control and Prevention 4. CDPHE—Colorado Department of Public Health and Environment 5. CLIA—Clinical Laboratory Improvement Amendments 6. Client-An individual receiving contracted services from the Contractor 7. Comprehensive STI Screening-Chlamydia,gonorrhea,syphilis,and HIV 8. DIS-Disease Intervention Specialist 9. Educational Sessions-Providing information and education to healthcare providers and priority populations about STIs,STI testing,STI treatment and STI referrals.Educational sessions may occur through community outreach. 10. FDA—Food and Drug Administration 11. FDT Eligibility Criteria-Any individual with a positive syphilis screen or anyone who is a sex partner to someone who has tested positive for syphilis. 12. FDT-Field-Delivered Therapy is the delivery of medication or a prescription to a client's home or other non-clinical setting 13. HCV-Hepatitis C Virus 14. HIV—Human Immunodeficiency Virus 15. Indicated Intervention—Comprehensive STI screening,treatment and referrals 16. Non Clinical Settings-These are settings where medical,diagnostic,and/or treatment services are not routinely provided.This includes testing at fixed venues or outreach sites,such as a person's home,community-based organizations,home-test kits,mobile testing units,churches, bathhouses,parks,shelters,syringe services programs,businesses,work release,and other social service organizations.Non-clinical settings are easily accessible and comfortable for populations who might not access medical services regularly. 17. Prenatal care-Medical care,specific to individuals who are pregnant,that is received during pregnancy; including check-ups and prenatal testing 18. Pregnancy Testing-A test that checks the client or patient's urine or blood for a hormone called human chorionic gonadotropin(HCG). 19. Quality Improvement Plan-A set of focused activities designed to monitor,analyze and improve the quality of processes in order to improve the outcomes of a program. 21. Rapid Syphilis Testing-point-of-care tests that can be performed outside a laboratory setting, using a small amount of whole blood collected by a finger stick. 22. Reactive rapid syphilis result-One of three possible results from a rapid syphilis test.A reactive result using a rapid syphilis test is also interpreted as`positive'. 23. REDCap—Research Electronic Data Capture:a secure web application for data collection 24. Reproductive Age- 15-45 years old 25. ROI-Release of Information 26. Screen-Assessment of clients to identify the possible presence of and/or risk for additional social, behavioral,and financial risks including,but not limited to,substance use,intimate partner violence,medication assistance,and linkage to care 27. Service area—Boulder, Larimer,Morgan,Weld,Washington,Philips,Logan,Yuma,and Sedgwick Counties 28. STI-Sexually Transmitted Infection 29. Support Services-Support services may consist of STI/HIV case management,linkage to care services,housing assistance,food services,mental health treatment,substance use treatment, transportation assistance,intimate partner violence services,crisis services,and medication assistance. 30. Technical Assistance(TA)-Guidance provided to LPHAs by sharing written protocols, reviewing processes and best practices,identification of key partnerships,and assistance with troubleshooting programmatic challenges.TA may be delivered virtually,through email and/or in person. 31. VH-Viral Hepatitis IV. Work Plan: Goal#1: Provide high-quality HIV,STI,and VH prevention services to increase the priority population's access to testing,education,and treatment,and reduce the acquisition of HIV/STINH throughout Colorado in order to disrupt health inequities. Objective#1:No later than the expiration date of the contract,reduce the spread of syphilis through a continuum of syphilis prevention intervention services for the priority population in the service area. Primary Activity#1 The Contractor shall complete the indicated intervention services. 1.The Contractor shall develop FDT protocols. 2.The Contractor shall implement FDT protocols. 3.The Contractor shall provide indicated interventions in non-clinical settings. 4.The Contractor shall offer confirmatory testing for clients with a reactive rapid Sub-Activities#1 syphilis result. 5.The Contractor shall offer pregnancy testing to females of reproductive age who test positive for syphilis. 6.The Contractor shall identify topics for technical assistance. 7.The Contractor shall offer field-delivered therapy to individuals who meet the FDT eligibility criteria. 8.The Contractor shall complete client-level data REDCap forms. Primary Activity#2 The Contractor shall evaluate patients for additional support services. 1. The Contractor shall screen patients for support services. Sub-Activities#2 2. The Contractor shall refer patients to eligible support services. 3. The Contractor shall review the results for each indicated intervention patient. The content of electronic documents located on CDPHE and non-CDPHE websites and information contained on CDPHE and non-CDPHE websites may be updated periodically during the Contract term. The Contractor shall monitor documents and website content for updates and comply with the most recent revisions. 2. The Contractor shall comply with the following rules and regulations when performing data collection and entry,reporting,and the implementation of activities. a. The CDPHE Communicable Reportable Conditions Guidelines are located on the following state of Colorado website.This information is incorporated and made part of this Contract for reference: https://www.colorado.gov/pacific/cdphe/report-a-disease b. The CDPHE STI and HIV funded partner guidance and policies are identified below.These documents are incorporated and made part of this Contract for reference and are available on the following website: https://www.colorado.gov/pacific/cdphe/sti-hiv-funded-partner-policies Standards and 3. The Contractor shall maintain one(1)of the following for syphilis testing Requirements throughout the life of the project: a. Valid,non-expired CLIA waiver certificate b. Valid,non-expired CLIA certificate 4. The Contractor shall use FDA approved testing technology when administering rapid syphilis testing and adhere to manufacturers' instructions as to use and storage. 5. The Contractor shall request technical assistance via email at cdphe sthv contractmonitoring@state.co.us. 6. CDPHE will provide a response to requests for technical assistance to the Contractor upon request within five(5)business days. 7. The Contractor shall collect a minimum of 90%of required data elements for Syphilis testing and other funded intervention services and activities. 8. CDPHE will act as the Contractor's data administrator for REDCap. 9. The Contractor shall provide access to REDCap for a minimum of one(1)staff person for the purposes of entering client level data. a. The Contractor shall complete the following REDCap instruments for the following client-level indicated interventions: a. Client Intake b. STI Testing c. HIV/HCV Testing d. FDT 11. The Contractor shall enter data related to contracted services into REDCap no later than ten(10)calendar days following each client encounter. 12. The Contractor shall use the required data elements described in the sample document NHM&E Data Variables and Values.This document is incorporated and made part of this Contract by reference and is available on the following website: https://www.colorado.gov/pacific/cdphe/sti-hi v-fun ded-partner-policies. 13. The Contractor shall use the REDCap Monthly Narrative Report form for the monthly progress reports. 14. CDPHE will provide the Contractor access to the REDCap Monthly Narrative Report form via REDCap within 30 days of Contract execution. 15. The Contractor shall include the following items in the Monthly Narrative Report for each respective contracted service: a. Description of how work was implemented for the contracted service b. Description of barriers or challenges related to providing the contracted service. c. Technical assistance needs d. Program changes 16. The Contractor shall submit distributable education materials created by funded staff for indicated interventions,by email to CDPHE for approval prior to implementation. a. The types of materials shall be submitted: i. Educational materials for contracted services. ii. Educational curriculum for contracted services. b. CDPHE will respond to material submissions within ten(10)business days and provide approval or required revisions. c. The Contractor shall edit materials as directed by CDPHE within ten(10) business days of receipt of receiving CDPHE feedback. 17. The Contractor shall provide the following active referrals for clients needing syphilis treatment. a. The Contractor shall work to refer patients for clinical treatment. b. The Contractor shall work to refer clients to field-delivered therapy(FDT) for those with a positive syphilis test that have barriers to treatment c. The Contractor shall work with CDPHE DIS to refer clients to treatment. 18. Quality Improvement Plan:The Quality Improvement Plan shall include the following related to the implementation and delivery of funded indicated intervention services and activities: a. Evaluate the number of individuals from the priority population screened for syphilis. b. Evaluate the syphilis positivity rate. c. Evaluate the efficacy of processes to ensure individuals from the priority populations with a positive syphilis screen that receive completed treatment. d. The Contractor shall submit the written draft Quality Improvement Plan to CDPHE Contract Monitor. e. CDPHE will provide feedback and make a request for any revisions to the Contractor within 15 business days of receipt. The Contractor shall complete the request for any suggested changes or revisions within 30 business days. g. The Contractor shall submit the final written Quality Improvement Plan. 19. The Contractor shall monitor the following: a. Evaluate the number of individuals from the priority population screened and treated in the following settings: i. Non-clinical settings ii. Field delivered therapy. b. Evaluate the efficacy of treatment processes to ensure individuals from the priority populations complete treatment. c. Monitor refusal rates for individuals who do not want syphilis screening. 20. The Contractor shall develop the following protocols for field-delivered therapy: a. Referral Procedure i. Referral forms b. FDT Consent form c. Anaphylaxis Standing Order d. FDT Quality Assurance Control Checklist e. Patient information handouts f. Patient Eligibility Criteria g. Patient Follow Up h. Data Documentation 21. The Contractor shall develop the following protocols for screening in non-clinical settings: a. Storage and handling of specimen samples and medication b. Testing procedures c. Standing orders d. Management of positive STI results e. Treatment procedures f. Referral procedures g. Management of contacts h. Coordination with DIS for clients with a positive syphilis screen 22. The Contractor shall maintain staff throughout the contract period who are responsible for the following activities: a. Draft and update protocols b. Provide indicated interventions in the clinical setting c. Provide indicated interventions in the non-clinical setting d. Complete the Quality Improvement Plan activities e. Complete data entry for indicated interventions f. Provide educational sessions g. Provide Technical Assistance to LPHAs h. Complete and submit monthly narrative reports 23. The Contractor shall substitute other employees to complete required activities in the case of employee vacancies. 24. CDPHE will provide a schedule for virtual monitoring calls with the Contractor,at a minimum,quarterly,within 45 days of Contract execution. 26. The Contractor shall submit deliverables to the CDPHE Contract Monitor via email at cdphe_sthv_contractmonitoring@state.co.us. Individuals at risk for acquiring syphilis will increase participation with syphilis Expected Results of prevention activities. Activity(s) 2. Individuals that have acquired syphilis will increase participation with syphilis treatment activities. Measurement of Expected 1. Number of clients served as tracked in REDCap. Results 2. Number of clients who are documented as adequately treated in REDCap. Completion Date 1. The Contractor shall submit a Monthly Narrative Report. No later than 30 calendar days following the last day of each month for months one(1) Deliverables through 11. 2. The Contractor shall submit Protocols. No later than six(6) months following the Contract start date. 3. The Contractor shall submit a draft Quality Improvement No later than six(6) Plan. months following the Contract start date. 4. The Contractor shall submit the final Monthly Narrative No later than the Report. Contract end date. 5. The Contractor shall submit the results of the Quality No later than the Improvement Plan. Contract end date. V. Monitoring: CDPHE's monitoring of this Contract for compliance with performance requirements will be conducted throughout the contract period by the CDPHE Contract Monitor. Methods used will include a review of documentation determined by CDPHE to be reflective of performance to include progress reports,REDCap reports or reports from other databases as designated by CDPHE,site visits,invoices and other fiscal and programmatic documentation as applicable.The Contractor's performance will be evaluated at set intervals and communicated to the Contractor.A Final Contractor Performance Evaluation will be conducted at the end of the life of the contract. VI. Resolution of Non-Compliance: The Contractor will be notified in writing within 15 calendar days of discovery of a compliance issue.
