Approve Standard Form for Amendment to Subrecipient Agreement for CDBG
Funds Between the Department of Finance and Various Subrecipients, and
Authorize Chair to Sign All Amendments Consistent with Said Form
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with a Standard Form for Amendment to
Subrecipient Agreement for CDBG Funds, between the Department of Finance and
Various Subrecipients, between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of
Finance, and various subrecipients, with further terms and conditions being as stated in
said standard form, and
Whereas, after review, the Board deems it advisable to approve said standard form, a
copy of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Standard Form for Amendment to Subrecipient Agreement for CDBG
Funds, between the Department of Finance and Various Subrecipients, between the
County of Weld, State of Colorado, by and through the Board of County Commissioners
of Weld County, on behalf of the Department of Finance, and various subrecipients, be,
and hereby is, approved.
Be it further resolved by the Board that the Chair be, and hereby is, authorized to sign
all amendments consistent with said standard form.
The Board of County Commissioners of Weld County, Colorado, approved the above
and foregoing Resolution, on motion duly made and seconded, by the following vote on
the 11th day of February, A.D., 2026:

PASS-AROUND TITLE: Standard Form for WC CDBG Program Amendment to Subrecipient Agreement

Brief description of the problem/issue: Approval of a Standard Form for all Amendment to Subrecipient
Agreements for use by the Weld County Community Development Block Grant (CDBG) Program is being
requested so these agreements may appear on the consent agenda, This would allow for quicker turnaround
times by eliminating the need to have such agreements appear on the Board of County Commissioners (BOCC)
agenda twice — once for presentation and one for approval. The proposed standard Amendment to the
Subrecipient Agreement represents an update to the current form being used to include a Tyler number to assist
the CTB in the handling of this document plus a few minor edits for clarification. The attached proposed form
has been reviewed by Bryron Howell, Assistant County Attorney, and found sufficient for use by the County's
CDBG Program.
On September 22, 2022, the BOCC approved by resolution the Standard Form for the Subrecipient Agreement
for CDBG funds between the Weld County CDBG Program and various subrecipients. In a similar manner,
approval of a Standard Form for Amendments to the Subrecipient Agreement is being requested.
What options exist for the Board?
1. Approve the use of the proposed Standard Form for Amendment to Subrecipient Agreements, a copy of
which is attached, and authorize the Chair of the Board of County Commissioners to execute individual
agreements between the Weld County CDBG Program and various subrecipients.
2. Do not approve the use of the proposed Standard Form. Amendments to Subrecipient Agreements would
be routed through the regular BOCC agenda.
Consequences: Option 2 uses more County resources.
Impacts: End result the same.
Costs (Current Fiscal Year/Ongoing or Subsequent Fiscal Years): None

AMENDMENT TO SUBRECIPIENT AGREEMENT FOR CDBG FUNDS
BY AND BETWEEN
THE WELD COUNTY CDBG PROGRAM AND THE [SUBRECIPIENT NAME]
THIS AMENDMENT to the Community Development Block Grant Subrecipient Agreement is entered into and made effective as of
this [DATE] of[MONTH], [YEAR] (the "Effective Date") by and between the Weld County Community development Block Grant
Program (herein called the "Grantee") and the [SUBRECIPIENT NAME] (herein called the "Subrecipient").
WHEREAS the Grantee and the Subrecipient entered into a Community Development Block Grant Subrecipient Agreement identified
as document No. [TYLER#] approved on [MONTH] [DATE], [YEAR] (the "Agreement")and;
WHEREAS both parties agree to amend the Subrecipient Agreement;
NOW THEREFORE the parties agree to modify the following parts of the Agreement:
