Approve Cooperating Team Member Agreement for Coordinated Investigations into
Child Abuse and Neglect Allegations, and Authorize Department of Human
Services, Sheriff's Office, and 19th Judicial District Attorney's Office to Sign —
Life Stories Child and Family Advocacy
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with a Cooperating Team Member Agreement
for the Coordinated Investigations into Child Abuse and Neglect Allegations among the
County of Weld, State of Colorado, by and through the Board of County Commissioners
of Weld County, on behalf of the Department of Human Services, the Sheriff's Office, and
the 19th Judicial District Attorney's Office, and Life Stories Child and Family Advocacy,
and various agencies, commencing January 15, 2026, and ending January 14, 2028, with
further terms and conditions being as stated in said agreement, and
Whereas, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Cooperating Team Member Agreement for the Coordinated
Investigations into Child Abuse and Neglect Allegations among the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, the Sheriff's Office, and the 19th Judicial
District Attorney's Office, and Life Stories Child and Family Advocacy, and various
agencies, be, and hereby is, approved.
Be it further resolved by the Board that Jamie Ulrich, Director of the Department of
Human Services, Steve Reams, Weld County Sheriff, and Michael Rourke, 19th Judicial
District Attorney be, and hereby are, authorized to sign said agreement.

Re: Life Stories Child Advocacy Center Cooperating Team Member Agreement
Life Stories Child Advocacy Center has issued an updated Cooperating Team
Member Agreement for Weld County multidisciplinary teams that provide
professional expertise in responding to allegations of child abuse and neglect.
The team includes, but are not limited to:
• Law enforcement agencies
• Child Protective Services
• Medical and mental health professionals
• Victim advocates
• Prosecutors from the Weld County District Attorney's Office
This updated agreement extends the term from January 15, 2026 through
January 14, 2028.
The Department requests approval for the DHS Director to sign the updated Life
Stories Child Advocacy Center Cooperating Team Member Agreement.

Mission
At Life Stories Child& Family Advocacy,we believe that every child should live a life free of
abuse in a safe, permanent home.We listen to stories and advocate on behalf of children
who have been abused and neglected so that our children will have a better tomorrow.
Vision
We promise to create a responsive community where every life story is honored and valued
as we advocate for children and families with compassion and respect.
Purpose
1. Coordination of the team approach to investigations involving but not limited to (a)
law enforcement personnel,(b)child protective services,(c) medical personnel, (d),
mental health, (e)victim advocates and(f)prosecutors from the District Attorney's
Office
2. To establish and maintain a nurturing, child-focused setting in which investigations
can be done.
3. To centralize resource materials for professionals and families.
4. To enhance professional skills necessary to effectively respond to child abuse
through training, education and collaboration.
5. To participate in activities which provide a comprehensive, multidisciplinary response
to child abuse; and
6. To provide families with support and necessary referrals
Scope of Services
• Coordination of forensic interviews including the use of an in-house forensic interviewer;
• The use of Life Stories facility both in Greeley and in Ft. Lupton;
• Audio/Video recording of all interviews;
• Coordination of non-acute medical examinations at the request of law enforcement;

• Appropriate referrals and follow-up on referrals for children and families including but
not limited to information on Crime Victim's Compensation,the Victims' Rights Act,
therapy and medical exams.
• Coordination of monthly case review staffings for the District Attorney's Office.
THIS AGREEMENT is made by and between the agencies and individuals involved in Life
Stories:
The purpose of this agreement is to enhance the ability of agencies, organizations, and
individuals to implement coordinated efforts in investigation of child abuse and neglect.
Adherence to these policies and procedures should not be construed as a creation of a
higher-level standard of safety or care with respect to any civil claim or criminal matter.
Therefore, it is acknowledged that the UNDERSIGNED participating agencies and individuals
agree to the following philosophical and policy statements supporting Life Stories:
We recognize the trauma that occurs to children and families involved in child abuse and
neglect. In order to reduce that trauma, Life Stories and key members of the
multidisciplinary team have developed a cooperative team approach to conducting child
abuse and neglect investigations.
To facilitate this team approach, Life Stories enhances the cooperation and coordination of
those involved in the protection of children. Life Stories offers a safe, child focused
environment to centralize the investigation and prosecution efforts for child victims of
abuse.
Each of the undersigned agencies and individuals has specific responsibilities for the
protection of victims and the investigation of child abuse which are outlined in their
individual policy and procedures and any joint agreements among team members. Use of
Life Stories does not alter specific role responsibilities.
Child abuse and neglect investigations require timely sharing of information in order to
coordinate services effectively.The team also agrees to uphold the highest standards of
confidentiality during the coordination of these cases. Information will not be shared
outside of the MDT without an additional signed written agreement in place. If the individual
or agency is not considered a part of Life Stories or the MDT(ex. Parents, Legal Aid, private
attorney, etc.)then a release must be ordered by subpoena.To ensure compliance will all
relevant statutes, rules and regulations, Life Stories consults with investigative parties prior
to sharing any forensically relevant information outside of the MDT.
Child abuse and neglect investigations require timely sharing of information to coordinate

forensically relevant information outside of the MDT. Information sharing is done for the
purposes of:
We believe that by functioning as a multi-disciplinary team,we are providing Weld County
children and families with a more positive approach that is effective in the timely resolution
of specific cases and the ultimate resolution of the larger social issue.
All parties are responsible for adhering to the Colorado Children's Code and the Colorado
Criminal Statutes.
All parties understand that this agreement, in no way, shall be construed to:
• limit law enforcement's authority to investigate reports of child abuse and
neglect;
• limit the Department of Human Services authority to assess, respond, and
investigate reports of child abuse and neglect nor limit DHS authority to make all
subsequent decisions on behalf of children placed in protective custody;
• dictate medical practitioner's procedures for medical examinations or limit their
authority or responsibility for medical care.
Therefore,to better protect and serve the children of Weld County, we agree to support the
concept, philosophy, policies, procedures, and continued development of Life Stories Child
Advocacy Center. Members of Life Stories multidisciplinary team agree to:
• Utilize Life Stories for at least 75%of the interviews of child victims of sexual
abuse,ages 2.5 to 17 years of age. Life Stories will be further utilized for severe
physical abuse, severe neglect,and witnesses to crime for children 2.5 to 17 years
of age,as appropriate. Life Stories will also interview 18-year-olds and adults
with developmental delays and special needs as determined by the forensic
interviewer and the investigating parties. No suspected perpetrators are allowed
at Life Stories. If a juvenile is a suspect and a victim,the juvenile will only be
interviewed at Life Stories as a victim at which time no other children are allowed
in the Child Advocacy Center.
• Review Life Stories Policies and Procedures and be signatories on the
Cooperating Team Agreement.
• Share and receive pertinent case information in a timely manner and in
adherence to relevant state laws.
• Contact families on their caseload to ensure continued communication
throughout the life of the case.

• Abide by all applicable confidentiality requirements of CR519-1-303 and
CRS 19-3-307.
• Send investigators to attend monthly Case Review staffings with the District
Attorney's Office.
This agreement will be signed every 3 years unless the following conditions arise:
• Changes are made to the Cooperating Team Agreement;
• There is a change in leadership in any of the signees. Only the new leadership will sign
and they will be added back to the 3-year signing schedule.
