1. PAYMENT TERMS:Payment terms will be pre-arranged with customer prior to delivery of equipment(C.O.D.and/or net 10 days from date
of invoice). Customer shall be liable for all fees or other charge applicable to this agreement or customers use of the equipment.
2. USE BY CUSTOMER:
A.Customer to inspect the equipment and find it to be in good condition and suitable for his use upon delivery.
B.Customer will permit the equipment to be used only for the purposes for which it was intended.
C.Customer will make no alterations or attachments to the equipment.
D.Customer has chosen the location for installing the equipment and accepts all responsibility in connection with that choice of location
IIincluding liability for any damage to underground utilities during the installation.
Customer will give United Site Services free access to the equipment at all times over suitable pavement or other driving surface
and will make the equipment available for maintenance at ground level without hazard to employees or equipment.
F.Customer will notify United Site Services immediately and discontinue use of the equipment if the equipment becomes unsafe or in a

4G.Customer will not permit the equipment to become subject to any lien,charge or encumbrance.
3. MAINTENANCE: United Site Services will service the equipment in accordance with the terms set forth on this agreement.
United Site Services obligation to maintain the equipment in good working order under ordinary use is conditioned upon customer's
compliance with the use obligations set forth in paragraph 2.
STOMER'S INDEMNITY:Customer will indemnify United Site Services,its employees and agents against any claim,liability,or cost's)
ing from this agreement or the use of the equipment,including property damage and personal injuries,except to the extent that such
aims,liabilities or costs are due to United Site Services sole negligence. Customer will promptly reimburse United Site Services for any
damage to or loss of the equipment. Equipment damage beyond repair will be paid for by the customer at replacement cost.
5. GENERAL:
A.Without the prior written consent of United Site Services,customer will not assign this agreement or any rights or obligations hereunder
and will not transfer possession or control of the equipment.
B.The provisions of this agreement can be waived or modified only by a writing signed by both parties. Failure by United Site Services to
enforce any provision shall not constitute a waiver of the provision. Acceptance of the returned units shall not waive any claims by United
Site Services against the customer.
C.United Site Services shall not be liable for any failure to perform caused by weather,strikes,shortage of materials or other causes
beyond its control.
D.The invalidity,unenforceability or waiver of any of the provisions shall not affect the remaining provisions.
6. ATTORNEY'S FEES:If any action be filed by any party to this agreement to enforce any of the terms and conditions of the agreement,the
prevailing party in such litigation shall be entitled to recover such additional sum as the court having jurisdiction in the matter may deem
reasonable as attorney's fees.

COMMERCIAL SERVICE AGREEMENT
This agreement is entered into by and between Customer listed above, (hereafter"Customer")and Gator Rubbish Removal LLC,
(hereafter"Hauler").
NOW,THEREFORE,FOR AND IN CONSIDERATION of the mutual promises and agreements contained herein,Customer hires
Hauler,and Hauler agrees to work for Customer under the terms and conditions hereby agreed upon by the parties:
1. Scope of Services. Customer agrees to hire Hauler to perform . trash collection;Hauler will
provide ,size . _ cubic yard dumpster(s)for the Customer to use for trash collection.
Customer agrees to not block or obstruct access to the dumpster at any time.Customer will agree not to allow children or
adults to play in or around the dumpster. Customer shall also agree to pay for damages caused by the Customer to the
dumpster.Upon completion of any pick-up,Hauler shall keep Customer's property in a clean and satisfactory condition.
Hauler agrees that at all times they will haul and dump debris in conformity with local,state,and federal laws,and that
debris will be dropped at a designated dump site.
A.. Term of Service. Hauler agrees to perform such services the performance of which shall begin and shall
remain in effect for a period of year(s).Customer agrees that unless canceled in writing,ninety(90)days prior
to the end of the term,this Agreement shall automatically continue for an additional year(s)term.
3. Payment.Customer agrees to pay Hauler, in consideration of the services contracted for,the sum of$_ per
month.Customer will be billed every month and payment is due prior to the start of service. Service prices will be
guaranteed for a period of one(1)year and shall increase up to six(6)percent each year thereafter. A finance charge will be
applied to all past due accounts. Excessive trash or large items will require an additional fee,Customer agrees to contact the
Hauler before these items are picked up.
4. Fuel Charges.Fuel charges will be included in the monthly price.
5. Additional Services.Customer agrees to the following additional services when the customer requests the service.
• This Agreement constitutes the entire agreement between the parties and shall not be supplemented,modified or amended unless
executed in writing by both parties.No waiver of any provision of this Agreement shall be deemed a waiver of any other
provision.This Agreement shall be construed according the laws of the State of Colorado,county of Weld.In the event that it
becomes necessary to enforce any of the terms of this Agreement the defaulting party agrees to pay all reasonable attorneys fees
incurred therein.Force Majeure: Hauler shall not be liable for any failure in performance due to an act of God,
weather, fire, sabotage, riot, explosion, or terrorism. Credits to your account will not be honored if any of the above
occurs and we are unable to collect Customer's trash. Hauler will collect all your trash on the next pick up date or as
soon as deemed possible.

CHANGE/EXPANSION OF USE OF AN EXISTING WELL Fasting: Northing:
ISSUANCE OF THIS PERMIT DOES NOT CONFER A WATER RIGHT
CONDITIONS OF APPROVAL
1) This well shall be used in such a way as to cause no material injury to existing water rights, The issuance of this permit does not ensure that
no injury will occur to another vested water right or preclude another owner of a vested water right from seeking relief in a civil court action.
2) The construction of this well shall be in compliance with the Water Well Construction Rules 2 CCR 402-2,unless approval of a variance has
been granted by the State Board of Examiners of Water Well Construction and Pump Installation Contractors in accordance with Rule 18.
3) Approved pursuant to CRS 37-90-137(4)and the findings of the State Engineer dated July 7,2010.
4) Approved for the change/expansion of use of an existing well constructed under permit no.269487. Issuance of this permit hereby cancels
permit no.269487.
5) The use of ground water from this well is limited to commercial uses.
6) The pumping rate of this well shall not exceed 15 GPM.

The average annual amount of ground water to be appropriated shall not exceed 2.8 acre-feet.
8) Production is limited to the Laramie Fox Hills aquifer.
9) The entire length of the hole shall be geophysically logged as required by Rule 9 of the Statewide Nontributary Ground Water Rules prior to
installing casing.
10) The owner shall mark the well in a conspicuous place with well permit number(s),name of the aquifer, and court case number(s)as
appropriate. The owner shall take necessary means and precautions to preserve these markings.
11) A totalizing flow meter must be installed on this well and maintained in good working order. Permanent records of all diversions must be
maintained by the well owner(recorded at least annually)and submitted to the Division Engineer upon request.
12) This well shall be located at least 600 feet from any existing well,completed in the same aquifer,that is not owned by the applicant,except
well permit no.89428,for which the well owner signed a 600 foot well spacing statement.
13) This well shall be located no more than 200 feet from the location specified on this permit.
14) Pursuant to CRS 37-90-137(9)(b)and the Denver Basin Rules, no more than 98%of the nontributary ground water withdrawn annually shall
be consumed and the well owner shall demonstrate to the reasonable satisfaction of the State Engineer that no more than 98%of the water
withdrawn will be consumed.
15) This well is subject to administration by the Division Engineer in accordance with applicable decrees,statutes,rules,and regulations.
NOTE:The ability of this well to withdraw its authorized amount of water from this non-renewable aquifer may be less than the 100 years
upon which the amount of water in the aquifer is allocated,due to anticipated water level declines.
NOTE:To ensure a maximum productive life of this well,perforated casing should be set through the entire producing interval of the
approved zone or aquifer Indicated above.
