Mr. Preap:
The Left Hand Water District currently serves the above-referenced account as a single-
family residential tap. In conjunction with the application of the Cambodian Cultural
Center to change the use of this property to a commercial application, the District will
approve and implement a change of tap classification and water rates when the proposed
change of use is approved by Weld County.
A commercial tap with our District will require the installation of an approved backflow-
prevention device, and annual testing of this device by licensed professionals. Enclosed
is information regarding the District's commercial backflow prevention requirements.

Dear Mr. Preap:
The following are the percolation test results and septic system evaluation for the existing
Cambodian Cultural Center. A maximum of 10 people will live in the center. Cultural events will
occur approximately 10 to 12 times a year with a maximum of 49 participants. Approximately
200 people attend the Cambodian New Year celebration which is held once a year. This
celebration is held outdoors in a tent and is a picnic/potluck style event. Portable toilets are used
• for the New Year celebration.
Percolation Test Results
Laube Engineering performed a soil percolation test at the subject site on September 15, 2007.
The average percolation rate of the existing site soils is 58 minutes per inch, which meets the
County criteria for percolation rates for a standard system. No limiting zones of groundwater or
impermeable layers were encountered.
Existing Information
The septic evaluation has been performed in accordance with Weld County Regulations and
Criteria — Chapter 30 Individual Sewage Disposal Systems. The evaluation utilizes the soil and
groundwater information provided by the percolation test.
The site is located approximately '/4 miles west of Weld County Road 7, approximately %2 miles
south of Weld County Road 4. Land use in the site vicinity consists of mixed housing
developments and agricultural land. The site is in flood zone "C" according to the Flood
Insurance Rate Map (Panel 9705 of 1075) dated September 28, 1990. Flood zone "C" includes
areas outside of the 100-year floodplain with minimal flooding. The existing soils are mapped as
Ulm soils (Weld County Soil Survey), which are soils formed in calcareous alluvial and eolian
(wind blown) sediments from shale.

• Existing Septic System
The existing septic system consists of a 1,500 gallon septic tank. The extent of the leach field
could be observed by the presence of green grass during our site visit. A few holes were hand
dug to verify the area of the leach field. The leach field is estimated to be 45 feet wide by 90 feet
long.
Septic System Evaluation
Our calculations indicate that a 1,500 gallon tank and a 2,433 ft2 trench absorption field are
required for the intended use. It is our opinion that the existing septic system consisting of a
1,500 gallon septic tank and an approximate 4,000 ft2 trench leach field is adequate for the
intended use.
Septic System Maintenance
We recommend that the septic tank be checked for accumulation of solids every two to three
years. Removing (pumping) solids from the septic tank when needed will prolong the life and
increase the effectiveness of your leach field. Septic tanks typically require cleaning every 3 to 5
years and should be performed by a licensed septic contractor.
We appreciate providing engineering services for you. The Weld County Health Department
retains the final approval authority for all I.S.D.S. evaluations. Any questions or comments

On October 31, 2006, an evaluation of the existing individual septic disposal system at:
595 County Road 7, Erie;
Section 33,Township 01 North,Range 68 West,
• was conducted by Stephen J. Wiatrowski, an Environmental Health Specialist of this depaituient.
There is not enough information provided to determine if the system is adequate.No visible signs of
failure were observed at the time of inspection. This evaluation is based on a final treatment capacity for
a 3 bedroom residence.
Be advised,neither the County of Weld nor any of its agents or employees undertake or assume any
liability to the owner of the above property, to any purchaser of the above property or to any lending
agency making a loan on the above property or in the report.
This inspection was conducted for the purpose of determining compliance with current regulations and
for detecting health hazards observable at the time of inspection. This does not constitute a warranty that
the system is without flaw or that it will continue to function in the future. Inspections requested during
periods of snow cover and high soil saturation may be of questionable value to potential buyers due to
adverse conditions. Evaluations based on Statements of Existing(S.O.E.)relies on information the
property owner provides, under oath, indicating current status of the system and representing to the best
of his/her knowledge the system is not failing to function properly.
