RE: APPROVAL OF AMENDED AGREEMENT BETWEEN THE WELD COUNTY HEALTH
DEPARTMENT ALCOHOL RECOVERY AND REHABILITATION CENTER AND
WELD MENTAL HEALTH CENTER, INC.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, an amended agreement between the Weld County Health
Department Alcohol Recovery and Rehabilitation Center and Weld
Mental Health Center, Inc. , has been presented to the Board of
County Commissioners of Weld County, Colorado for approval , and
WHEREAS , the agreement entered into in January, 1980 has
been amended in the following areas :
1. Emergency Services
2. Non-Emergency Basis
3. Transportation
4 . Coordinators as Liaison with the
corresponding agency.
WHEREAS, the Board of County Commissioners has studied the
amended agreement and deems it advisable to approve the same.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the amended agreement
between the Weld County Health Department Alcohol Recovery and
Rehabilitation Center and Weld Mental Health Center, Inc. be,
and hereby is , approved.
BE IT FURTHER RESOLVED by the Board that the Chairman of
the Board of County Commissioners be, and hereby is, authorized
to sign the amended agreement.

AFFILIATION AGREEMENT --
This AFFILIATION AGREEMENT is entered into this 1st day of January, 1980 by and
between WELD COUNTY HEALTH DEPARTMENT, Alcohol Recovery and Rehabilitation Center
and Weld Mental Health Center, Inc.
- In order to achieve cooperation, coordination, and communication in planning
matters of mutual concern, the Weld County Health Department and the Weld Mental
Health Center have determined that certain understandings shall take place between
them, namely:
1. The Weld County Board of Commissioners has recognized the Weld County
Health Department as the official agency for the establishment and operation
of the Alcohol Recovery and Rehabilitation Center which provides alcoholism
services to the residents of Weld County, including detoxification, outpatient
counseling, residential care, evaluations emergency services, D.U.Z. , other
court-related alcohol programs, monitored antabuse and consultation services.
The Alcohol Recovery and Rehabilitation Center is located at Island Grove
Park, Greeley. The Weld County Health Department, Alcohol Recovery and Rehab-
ilitation Center is licensed by the Colorado Department of Health as an alco-
holism treatment facility as provided by Weld County in the State of Colorado.
2. The Weld Mental Health Center, Inc. is a comprehensive community mental health
center providing mental health care, including drug abuse services, to the
residents of Weld County. The services include inpatient psychiatric care,
outpatient counseling, partial hospitalization, emergency services and consulta-
tion services to the community. The Weld Mental Health Center is certified
by the State Department of Institutions as provided under state and federal law.
3. The Weld County Health Department, Alcohol Recovery and Rehabilitation Center
will accept referrals from the Weld Mental Health Center of those county
residents appropriate to the alcoholism services of the Health Department.

The fee for these services will be the responsibility of the client, The
Weld Mental Health Center will accept referrals from the Weld County Health
Department, Alcohol Recovery and Rehabilitation Center of those clients
determined to be appropriate to the services of the Weld Mental Health Center.
Referrals will consist of a staff member of one agency maintaining contact
with an appropriate staff member of the other agency until the referral has
been completed. The receiving agency will determine which treatment modality
will be provided to the client. Fees for this service will be the responsi-
bility of the client.
4. Emergency Services.
Both agencies recognize the futility of making an assessment of an intoxicated
client to determine whether the person's primary problem is psychiatric or
alcohol. Therefore, no attempt will be made to delineate etiology until the
intoxication is resolved. Island Grove Center recognizes and agrees that it
shall respond to all emergency referrals and situations involving intoxicated
clients. If Weld Mental Health Center is initially contacted, Weld Mental
Health Center will refer the party to Island Grove, once the presence of
alcohol is established. If the intoxicated party refuses voluntary treatment
and otherwise meets the criteria of emergency situation as defined in 25-1-310
CRS 1973, a 5 day alcohol hold and treat shall be initiated. In no circum-
stances shall an involuntary intoxicated person be placed on a 72 hour mental
health hold and treat. Weld Mental Health Center recognizes and agrees that
it shall respond to a mental health crisis situation when the person needing
services is not intoxicated. When a person is identified as mentally ill and
meeting the criteria of 27-10-105, a 72 hour hold-and treat shall be initiated.
If Island Grove Center is initially contacted regarding a non-intoxicated
client, Island Grove shall make immediate referral to Weld Mental Health Center.

5. Non-Emergency Evaluations. Both agencies recognize that mutual evaluations
may be required in order to determine appropriate modeality of treatment.
Both agencies agree to request these evaluations through the on-call system
of the other agency. The referring and receiving clinicans shall mutually
decide whether a client needs to be interviewed in person. Evaluations shall
be scheduled and occur during regular working hours (8:00 a.m. - 8:00 p.m.).
6. Island Grove Center has the capability to transport clients in emergency and
non-emergency situations. Island Grove will transport clients to and from
Weld Mental Health Center to its facility when requested by Weld Mental
Health Center clinicians. When clients cannot provide their own transportation,
Island Grove will transport them to and from its facility to Weld Mental
Health Center. Requests for transportation for intoxicated clients are also
available and may be initiated by Weld Mental Health Center clinicians.
7. The Weld Mental Health Center will not be responsible for providing inpatient
care at Weld County General Hospital to alcoholism clients except within the
terms of its contract with Weld County General Hospital and as reimbursed under
that contract.
8. The Weld Mental Health Center and the Alcohol Recovery and Rehabilitation
Center will routinely request of clients who are being referred to the other
agency a signed release of information in order to expedite the exchange of
information so that an appropriate plan of treatment can be developed for the
client by the receiving agency.
9. Each agency agrees to designate a coordinator who will be responsible for
expediting referrals and acting as a liaison with the corresponding agency.
These coordinators agree to meet on a regular basis to insure the close coopera-
tion of the two agencies in providing care to the residents of Weld County.
Coordinators will jointly make sufficient community contacts to insure that
the referral process works smoothly.

10. This agreement may be amended by mutual consent of both parties at any'
time and may be cancelled by either party giving 30 days written notice to
the other party or any other mutually agreeable to both parties.
This agreement has been executed in triplicate, each of which shall be an
original.
