CONDMONS OF APPROVAL
1) This well shall be used in such a way as to cause no material injury to existing water rights. The issuance of this permit
does not assure the applicant that no injury will occur to another vested water right or preclude another owner of a vested
water right from seeking relief in a civil court action.
2) The construction of this well shall be in compliance with the Water Well Construction Rules 2 CCR 402.2,unless approval
of a variance has been granted by the State Board of Examiners of Water Well Construction and Pump Installation
Contractors in accordance with Rule 18.
3) Approved pursuant to CRS 37-92-602(3)(b)(II)(A)as the only well on a tract of land of 40.00 acres described as that portion
of Secs.27 and 34,Twp.6 N,Rng.64 W,Sixth P.M.,Weld County,more particularly described on the attached exhibit A.
4) The use of ground water from this well is limited to fire protection,ordinary household purposes inside not more than
three(3)single family dwellings,the irrigation of not more than one(1)acre of home gardens and lawns,and the watering
of domestic animals.
5) The maximum pumping rate of this well shall not exceed 15 GPM.
6) The return flow from the use of this well must be through an individual waste water disposal system of the
non-evaporative type where the water is returned to the same stream system in which the well is located.
7) This well shall be constructed not more than 200 feel from the location specified on this perrnit. 74/7/00

On 05-15-2002 an evaluation of the existing individual septic disposal system at:
27861 CR 62 3/4, Gill;
Section 34, Township 06, Range 64,
was conducted by Stephen J Wiatrowski, an Environmental Specialist of this department.
The existing individual septic disposal system is of sufficient size and capacity to adequately
handle the proposed load. This evaluation is based on a final treatment capacity for a 3
bedroom residence.
Be advised, neither the County of Weld nor any of its agents or employees undertake or
assume any liability to the owner of the above property, to any purchaser of the above property
or to any lending agency making a loan on the above property or in the report.
This inspection was conducted for the purpose of determining compliance with current
regulations and for detecting health hazards observable at the time of inspection. This does not
constitute a warranty that the system is without flaw or that it will continue to function in the
future. Inspections requested during periods of snow cover and high soil saturation may be of
questionable value to potential buyers due to adverse conditions. Evaluations based on
Statements of Existing (S.O.E.) relies on information the property owner provides, under oath,
indicating current status of the system and representing to the best of his/her knowledge the
system is not failing to function properly.

The issuance of this permit does not imply compliance with other state,county or local regulatory or building requirements, nor
shall it act to certify that the subject system will operate in compliance with applicable state, county and local regulations adopted
persuant to Article 10,Title 25, CRS as amended,except for the purpose of establishing final approval of installed system for
issuance of a local occupancy permit persuant to CRS 1973 25-10-111 (2).
This permit is not transferable. The Weld County Department of Public Health and Environment reserve*the right to impose
additional terms and conditions required to meet our regulations on a continuing basis. Final permit approval was contingent upon
the final inspection of the completed system by the Weld County of Public Health and Environment.
