Subject: Conquest 3, Conquest Disposal Service,Ltd., Offloading Pad Modification Approval
Dear Mr. Goddard:
The Weld County Department of Public Health and Environment (the Department) has reviewed the
Offloading Pad Modification for the Conquest 3 injection well, Weld County, Colorado. Randy Price of
CGRS prepared the modification notification dated August 25, 2004.
The modification design documents indicate the offloading pad will consist of a soil vapor extraction
system underneath the secondary containment and concrete receiving pad. The secondary containment
will consist of a seamless 40 mil High Density Polyethylene and petroleum resistant liner under the
concrete receiving pad. The edges of the liner will be turned up and poured into the concrete pad. The
new trough will consist of a prefabricated, interlocking, chemical resistant,polyester trench drain system.
As a result,the Department has no concerns with the modification documents as submitted.
The offloading pad modification appears to be in compliance with the facility's Special Review Permit
(USR-1198). As a result, the Department has no concerns with the receiving pad modifications as
submitted.
Additionally, as discussed the petroleum impacted soils are to be remediated on site. Evidently this
activity has been approved by Randall Ferguson of the Colorado Oil and Gas Conservation Commission.
Please provide a written description of the location of the remediation area, containment plans, and a
timeline that it will take to complete remediation project and remove the petroleum impacted soils from
the site.
