The above referenced account is coming up for renewal December 31, 2002. In order to
comply with the Nevada statute, this letter shall serve as notice of possible changes in
the terms and conditions of the above referenced policy. Upon renewal, these changes
may include a higher retention and/or an increase of up to, or more than 20% of the
premium.
Midwest Employers Casualty Company appreciates your business and we look forward
to offering you our renewal proposal. Prior to being able to offer renewal terms and
conditions, we will need the attached renewal application completed. Please pay
particular attention to:
i) Projected payroll by class code for the upcoming renewal term.
2) Paid and incurred amounts for all large losses having a total incurred value
of$50,000 or more for the last five years.
3) Total paid and incurred losses by policy year for at least the last five years.
4) Total claim counts excluding claims closed without payment by policy year
for the last five policy years.
5) Complete attached Employer Profile.
6) Complete attached Aircraft Supplemental Application.
Note: It is now required that all new and renewal applications be signed by the insured.
Items (2) through (4) should be provided as of the same evaluation date.
