The purpose of this inspection was to review all construction/operational aspects of this facility and compare the constructed facilities against the commitments made and requirements recommended for this operation. Attached is an inspection list utilized during the inspection. It was found that the facility has complied with a majority of these issues. However, four items were found which have not been complied with per the applications submitted and the recommendations listed in the issued certificate of designation. These items are: 1 ) The applicant has not provided a $500,000 bond for his facility operator, 2) No $100,000 Oil and Gas Conservation Commission bond has been posted for the operation of this facility, 3) At the time of this inspection no permanent operating record book was available, and 4) The plans for the construction of the skim tanks, dump tanks and small peripheral drain systems around these areas have not been provided. All other "as-built-plans" have been provided in a report dated November 5, 1984. Based upon this inspection and review of the documents submitted, it is this Division's opinion that this facility is substantially in compliance with the state regulations at the present time. Items 1 and 2 above were required by Weld County and are not within our purview.
