RE: APPROVAL OF ADDENDUM #4 TO THE AGREEMENT FOR SERVICES BE-
TWEEN WELD COUNTY AND GALLAGHER BASSETT INSURANCE
WHEREAS , the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS , Addendum #4 to the Agreement for Services between
Weld County and Gallagher Bassett Insurance Service has been
presented to the Board of County Commissioners for approval, and
WHEREAS , said Addendum concerns an amendment to Item III
(Compensation) of said Agreement, and
WHEREAS , after review, the Board deems it advisable to
approve said Addendum, a copy of which is attached hereto and
incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that Addendum #4 to the
Agreement for Services between Weld County and Gallagher Bassett
Insurance Service be, and hereby is , approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be ,
and hereby is, authorized to sign said Addendum #4 .
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 5th day of
March, A.D. , 1984.

ADDENDUM #4
TO
AGREEMENT FOR SERVICES
BETWEEN
BOARD OF COMMISSIONERS OF WELD COUNTY, COLORADO
AND
GALLAGHER BASSETT INSURANCE SERVICE, A DIVISION OF ARTHUR 3. GALLAGHER& CO.
PERIOD OF AGREEMENT: January 15, 1980 - Continuous
DATE OF CHANGE: January 15, 1984
It is hereby noted and agreed that Item III (Compensation) of this Agreement is
amended to read:
"EMPLOYER shall pay SERVICE AGENT for services rendered herein the
annual sum of SIXTY-SIX THOUSAND FOUR HUNDRED SEVENTY-FIVE
DOLLARS ($66,475.00). Payment is due within thirty (30) days of each
annual renewal date. At the end of each year of this Agreement, the
annual compensation shall be subject to adjustment as mutually determined
by EMPLOYER and SERVICE AGENT, based on changes in workload.
SERVICE AGENT agrees to notify EMPLOYER in writing of any increase in
service fee based on changes in workload thirty (30) days prior to each
anniversary."
All other terms and conditions remain unchanged.
