RE:: APPRO TEN PURCHASE OF SERVICES AGREEMENTS WITH VARIOUS
PROVIDER AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the oard of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the eld County Home Rule Charter, is vested with the authority of
administering the affairs ofWeld County, Colorado, and
WHEREAS, the Board ha been presented with ten Purchase of Services Agreements;
between the County of Weld, State f Colorado, by and through the Board of County
Commissioners of Weld County, on half of the Department of Human Services, Family
Educational Network of Weld County, d the following providers:
1) Mesa County Department o Human Services
2) Colorado Department of Pub ' Health and Environment
3) Two with Weld County School istrict RE-3J (Meals and Preschool Program)
4) Two with Weld County School Di trict RE-5J (Meals and Preschool Program)
5) Boulder County School District R 1J
6) Plan de Salud del Valle ,
7) Trinity Housing Corporation of Greele\
8) Skyline Dental
WHEREAS, after review, the Board deems it advis le to approve said agreement>,
copies of which are attached hereto and incorporated herein y reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of ounty Commissioners of
Weld County, Colorado, that the ten Purchase of Services Agree nts between the County of
Weld, State of Colorado, by and through the Board of County Com 'ssioners of Weld County,
on behalf of the Department of Human Services, Family Educational etwork of Weld County,
and the above listed providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements. \
