RE: APPROVE MEMORANDUM OF AGREEMENT FOR A PEER INTERVENTION
PROJECT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Agreement for a
Peer Intervention Project between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing January 3, 2000, and ending June 30, 2000, with further terms and
conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Memorandum of Agreement for a Peer Intervention Project
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 19th day of January, A.D., 2000, nunc pro tunc January 3, 2000.

SUBJECT: Memorandum of Understanding for Peer Intervention Project
DATE: January 17, 2000
Enclosed for Board review and approval is a memorandum of understanding between Weld
County and Colorado Department of Public Health and Environment for the Family Planning
Program Peer Intervention Project.
The Health Department has been awarded funding in the amount of$5,000 for this project
for the time period January 3, 2000 through June 30, 2000. The purpose of the project is
teen pregnancy prevention using peer-based education and support to high-risk youth in both
one-on-one and group settings. The peer education is comprehensive to not only include facts on
teen pregnancy, abstinence, and other birth control options, but also HIV/STDs, date rape/sexual
assault, communication and negotiation skills, self-esteem, and community resources.
The $5,000 will provide for three hours per week of a health educator's time to coordinate the
program and it will provide incentives for the peer educators. Educational materials and the cost
of training are also included in the budget for this program.
Health Department staff recommends your approval of this memorandum of understanding.

This document details the services to be provided as part of the Family Planning Program-
Reduction in Unintended Pregnancy Initiative. Weld County Health Department has been
awarded $5,000 to provide services outlined in the submitted proposal and revised objectives.
The agency will also maintain statistical information for assessment of the project as listed in the
proposal.
Funding for the project begins January 3, 2000 and ends June 30, 2000. Billing will be
submitted for$2,500 at the February 26, 2000 midpoint and at the June 30, 2000 completion date
for the remaining $2,500 unless other arrangements are made. A midpoint project update is due
February 26, 2000 and a final report is due July 31, 2000.
