RE:: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT FOR USE OF
SPACE IN GRAND JUNCTION, COLORADO, FOR MIGRANT HEAD START
PROGRAM AND AUTHORIZE CHAIR TO SIGN - INTERMOUNTAIN ADVENTIST
ACADEMY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment for use of space in Grand Junction, Colorado, for the Migrant Head Start Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Family
Educational Network of Weld County, and the Intermountain Adventist Academy, commencing
July 24, 2000, and ending October 27, 2000, with further terms and conditions being as stated
in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement Amendment for use of space
in Grand Junction, Co between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, Family Educational Network of Weld County, and the Intermountain Adventist
Academy be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 31st day of July, A.D., 2000, nunc pro tunc July 24, 2000.

SUBJECT: Purchase of Services Agreement Amendment
between the Family Educational Network of
Weld County and Intermountain Adventist
Academy
Presented before the Weld County Board of County Commissioners and Intermountain
Adventist Academy is a contract amendment, for space in Grand Junction. The
amendment is an extension of time for space rental until October 27, 2000. The cost of
rent will be $125.00 per week plus $30.00 per month for trash removal.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 3iSTday of July, 2000 by and between the
Weld County Division of Human Services' Family Educational Network of Weld County,
Migrant Head Start Program, hereinafter referred to as "Lessee", and , Intermountain
Adventist Academy, hereinafter referred to as "Lessor."
WITNESSETH:
WHEREAS, the"Lessee"desires to continue renting space from the lessor, located at 1704
N. 8`h St., Grand Junction, CO 81501, and
WHEREAS, the "Lessor" and "Lessee" are desirous of amending the original agreement of
June 14,2000.
NOW THEREFORE, it is mutually agreed:
1 . The "Lessee" will reimburse the "Lessor" $125.00 per week, for use of the gym, plus
$30.00 per month for trash removal.
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. The terms of this Amendment extend from July 24, through October 27, 2000. T his
Amendment will be terminated immediately in the event of loss of funding.
IN WITNESS WHEREOF,the parties hereto have executed this Amendment on the day first
written above.
