RE: APPROVE COMMUNITY GRANTS PROGRAM APPLICATION FOR COMMUNITY
HEALTH NEEDS ASSESSMENT AND AUTHORIZE CHAIR TO SIGN -AMERICAN
CANCER SOCIETY, ROCKY MOUNTAIN DIVISION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Community Grants Program
Application for Community Health Needs Assessment from the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, to the American Cancer Society,
Rocky Mountain Division, with terms and conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Community Grants Program Application for Community Health
Needs Assessment from the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, to the American Cancer Society, Rocky Mountain Division, be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said application.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 2nd day of February, A.D., 2000.

Subject: Grant application to the American Cancer Society, Rocky Mountain
Division, for Community Health Needs Assessment
Enclosed for Board review and approval is a grant application to the American Cancer
Society, Rocky Mountain Division. The grant will provide funds to conduct a county wide
community health needs assessment. The Centennial BOCES will collaborate with WCDPHE in
this process.
A comprehensive assessment is a means of collecting information to identify the major
health concerns in Weld County . This information will assist the community leaders in Weld
County, WCDPHE, American Cancer Society, the Centennial BOCES and many other agencies
with:
1. Short and long range strategic health planning
2. Insuring the best use of limited resources to address these concerns
3. Creating support in the county "community" for needed changes
The residents of Weld County are more likely to buy-in and support these plans if it is their
opinion that helped identify and prioritize the issues.
The term of the grant , if received, would be March 1,2000 - February 28, 2001 and the
community health needs assessment process takes approximately 1 year to complete. I recommend
your approval.

Proposed Program:
The mission of the Weld County Department of Public Health & Environment
"WCDPHE" is to preserve, promote and protect a healthy life and safe environment for the
residents of Weld County. To accomplish this, WCDPHE proposes to conduct a county wide
Community Health Needs Assessment "CHNA" which addresses the personal, consumer and
environmental health of the residents.
The CHNA includes a comprehensive survey, focus groups and key informant interviews.
The data that is generated will give our agency, American Cancer Society "ACS" and all of our
communities the information needed to address the priority health concerns. The most recent
data review by WCDPHE was done in 1994, Health Objectives for Year 2000, which has served
as a public health planning document for the past 6 years. To date, a comprehensive county wide
health needs assessment has not been done.
The CHNA will be constructed so each segment of the process includes questions that
address the Nationwide Cancer Control Objectives. All questions, including those about the
quality of life objectives, will be developed with input from cancer survivors (volunteers) and
ACS. The ACS Nationwide Objectives address colon/rectal, lung, breast, prostate and skin
cancer; youth tobacco use and comprehensive school health education and preventive nutrition.
These same objectives are priorities in Weld County as well. In Weld County in 1997,
1,023 individuals died, and of these, 27.3 % died as a result of cancer. Weld County and
Colorado differ from the national trend because the number one killer is cancer followed by heart
disease with the converse the case at the national level. In Weld County in 1995-96, the age-
adjusted incidence rate of lung cancer in men was 79.5/100,000, significantly higher than
Colorado's rate, 63.5/100,000. During 1996-97, the age adjusted mortality rate for men in Weld
County was 59.7/100,000 while Colorado's rate was 47.2/100,000 with lower rates in both years
for women.
The stakeholders for the CHNA will include The American Cancer Society Rocky
Mountain Division and representatives from Weld County cities, communities, businesses,
schools, and other agencies. The Stakeholders may assist with/or approve questions for survey
content, focus groups and key informant interviews and provide suggestions for recruiting key
informants and focus group participants.
The Centennial Board of Cooperative Educational Services "BOCES", which serves
8,000 children ages 3 - 21 years in 8 of the 12 school districts in Weld County, will collaborate
with the health department to develop and carry out the assessment. They will also assist with
obtaining more complete assessment data from the rural communities in the county.
WCDPHE, ACS, BOCES and other agencies can establish baseline data and indicators
for program development and resource allocation using a CHNA to identify and address the
health concerns of Weld County citizens. At the conclusion of the CHNA, WCDPHE will
provide the Stakeholders with a comprehensive report of the quantitative and qualitative data that
documents the health of the residents of Weld County. The stakeholders will know what the
leading health concerns are, what people like and dislike, its strengths and weaknesses and what
resources are available in the county. The CI-NA will directly address most of the gaps identified
in the ACS Assessment Summary Weld County, 1999-2000. Weld County citizens are much
more likely to support these approaches to address health concerns because it is their opinion that
empowers them to make needed changes.
Capability:
WCDPHE, established in 1938, serves a population of, according to the 1999 estimates,
164, 348 residents and encompasses a geographic area of 4004 square miles. The largest ethnic
group in Weld County is Hispanic, comprising 25% of the population. Using the 1997 US
census projection estimates, the number of residents under 25 is 65,139; the number of people
ages 25-44 is 46,017 and the number of people over age 45 is 54,234.
WCDPHE employs 81 individuals who work in Public Health Education and Nursing.
Environmental Health and Administration and over 30% are Hispanic/Latino. WCDPHE is
committed to providing and improving services to the non-English speaking clients. The
WCDPHE is actively recruiting members of the Hispanic/Latino community for an advisory
board to better understand and serve these communities. Hispanic representatives from several
Greeley and Weld County agencies are already involved in the HIV/AIDS coalition and Teen
Pregnancy Prevention Coalition. In addition, we provide accommodations for services to the hard
of hearing, deaf or disabled by ensuring access to location and interpretation from the Northern
Colorado Center on Deafness.
The WCDPHE supports the activities of the American Cancer Society in several ways.
WCDPHE staff members have served as committee volunteers and participated as team members
representing the health department in the Weld County Relay for Life race. The WCDPHE's
Colorado Women's Cancer Control Initiative coordinator implements the Tell-A-Friend program
in Weld County. The Tobacco Free Weld County "TFWC" coordinator participates in the C-
KIDS Project in the Greeley Evans School District Six. The TFWC Coalition is addressing adult
and youth tobacco issues in Weld County with input from UNC, City of Greeley Youth Initiative
and Greeley City Council, District Six and community volunteers.
The WCDPHE has developed a model for Community Health Needs Assessment using
nationally recognized standards. We have piloted CHNAs in several small communities in
Weld County during the past two years. The results have produced educational programs for
cancer control and prevention of teen pregnancy and substance abuse as well as paved roads,
ordinances banning burning of residential waste and improved waste collection services. Due to
budget constraints, the county was not able to provide funds for the comprehensive county wide
assessment in 2000.
Target Audience: The residents of Weld County, representing all age and ethnic groups are the
target audience. Volunteers from among the 4600 cancer survivors in Weld County will be
targeted as a sub-group of the population to participate in the survey, survey development, focus
groups and key informant interviews
CHNA Project Coordinator: WCDPHE has a .5 FTE Health Education Specialist to assist with
the Community Health Needs Assessment. The Health Promotion"HP" Supervisor will
coordinate the project. Other WCDPHE staff and volunteers are listed in the next section.
