RE: APPROVE CONTRACT CHANGE ORDER LETTER#3 FOR STATE PER CAPITA
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Change Order Letter#3 for
the State Per Capita Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing September 1, 2000, and ending June 30, 2001, with further terms
and conditions being as stated in said change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Change Order Letter#3 for the State Per Capita Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following voile on the 6th day of September, A.D., 2000, nunc pro tunc September 1 ,
2000.

Enclosed for Board review and approval is the State Per Capita Contract Renewal Letter No
between the Colorado and Weld County Departments of Public Health and Environment.
The State per capita appropriation for Weld County is $218,036 for the time period Septembe
1, 2000 through June 30, 2001. This is $8,386 or 4% more than we received last yea to to :al
public health efforts.

Pursuant to paragraph#9 of the contract with contract routing number 9807021 and contract encumbrance number
PO FAA ADM 9807021, (as amended by Contract Renewal Letter 3 contract routing number 00748,hereinafter
referred to as the"Original Contract"(a copy of which is attached hereto and by this reference incorporated herein
and made a part hereof)between the State of Colorado,Department of Public Health and Environment and
Weld County Department of Public Health&Environment,for the renewal term from September I,2000,
through June 30,2001,the parties agree that the maximum amount payable by the State for the eligible services
referenced in paragraph 7 of the Original Contract is increased by TWO HUNDRED EIGHTEEN THOUSAND
THIRTY-SIX DOLLARS (S218,0361 for a new total financial obligation of the State of EIGHT HUNDRED
THOUSAND ONE HUNDRED SEVENTY-EIGHT DOLLARS($800,178). The revised budget, which is
attached hereto as "Attachment 1", is incorporated herein by this reference and made a part hereof. The first
sentence in paragraph 7 of the Original Contract is hereby modified accordingly. All other terms and conditions of
he Original Contract are hereby reaffirmed. This amendment to the Original Contract is intended to be effective as
of September 1,2000. However, in no event shall this amendment be deemed valid until it shall have been
approved by the State Controller or such assistant as he may designate.
