HomeMy WebLinkAbout20000345 RESOLUTION RE: APPROVE COMMUNITY COLLABORATION AGREEMENT/MEMORANDUM OF UNDERSTANDING FOR WELD COUNTY PARTNERS PROGRAM AND AUTHORIZE CHAIR TO SIGN -WELD COUNTY PARTNERS WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Community Collaboration Agreement/Memorandum of Understanding for the Weld County Partners Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Weld County Partners, commencing July 1, 2000, and ending June 30, 2001, with further terms and conditions being as stated in said memorandum of understanding, and WHEREAS, after review, the Board deems it advisable to approve said memorandum of understanding, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the Community Collaboration Agreement/Memorandum of Understanding for the Weld County Partners Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Weld County Partners be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said memorandum of understanding. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 9th day of February, A.D., 2000. PURPOSE (What and Why) - Provide volunteer long term mentoring services through Weld County Partners to identified at-risk youth clients of Weld County Social Services. II. AGENCY RESPONSIBILITIES (Who and What) - 1. Social Services case workers will make available Partners' brochures at their agency and identify youth clients who voluntarily want to participate in the Partners Program. 2. Social Service case workers will complete referral forms with identified youth and their family and send the completed referral to Partners. All rules of confidentiality will be observed by Social Services and Weld County Partners. 3. Social Service case workers will communicate with Partners staff and the Senior Partner to provide the most appropriate services to the youth and their families. All rules of confidentiality will be observed by Social Services and Weld County Partners. III. SHARED CONCERNS (Why) - Both Partners and Weld County Social Services share a common concern regarding the need for additional support services (mentoring) for the at-risk youth that Social Services works with. IV. SUBSTANCE OF AGREEMENT (What) - Social Services will provide referrals to Partners for youth clients whose case workers have identified as appropriate and who would benefit from the Partners program. Partners will match the Social Service youth with a screened, trained, adult volunteer based on the youth and the adults common interests. V. LIAISON STAFF(Who) -Gloria Romansik, VI.
