RE:: APPROVE LETTER OF INTENT FOR TELEPHONE SERVICE FOR FORT LUPTON
GRADER SHED AND AUTHORIZE CHAIR TO SIGN - US WEST
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Letter of Intent for telephone service
for the Fort Lupton Grader Shed between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Phone Services, and US West, with terms and conditions being as stated in said letter of intent,
and
WHEREAS, after review, the Board deems it advisable to approve said letter of intent, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Letter of Intent for telephone service for the Fort Lupton
Grader Shed between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Phone Services, and US
West be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said letter of intent.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 6th day of November, A.D., 2000.

LETTER OF INTENT _
The Federal Communications Commission(FCC)provides guidelines for telephone serving arrangements. This letter serves as
documentation of the intended building use and of the service configuration to be provided by U S WEST Communications.

The Premises Owner or Designee must select one of the serving arrangements described below.
nI. U S WEST Communications facilities will terminate at one mutually agreed upon location upon entry into or on the
building,normally at the lowest common serving point. The building owner and/or tenants will provide,manage and
maintain building cable and wire placed beyond the Demarcation Point location.
2. U S WEST Communications will terminate facilities at common locations mutually agreed upon by the Premises
Owner or designee throughout the building(terminal rooms,utility closets,etc.). U S WEST Communications will
provide,manage and maintain the building cable and registration jacks that denote the Demarcation Points. The
Demarcation Points will be accessible to end-users at these locations. (Option 2 is not an option for single tenant
buildings.)
n 3. U S WEST Communications will terminate facilities at one mutually agreed upon location,within the individually
occupied space/unit(s),or within 12"or a similarly reasonable distance of cable/wire entry. U S WEST will provide,
manage and maintain the building cable,network terminating wire and registration jacks that denote the Demarcation
Point. (Option 3 is not an option for single tenant buildings.)
4. U S WEST Communications will terminate facilities at one mutually agreed upon location on the property. The
Premises owner and/or tenants will provide,manage and maintain building wire and cable placed beyond the
Demarcation Point Location.
It is the responsibility of the Premises Owner to provide and maintain adequate termination space and supporting structures for
telephone cable/wire within the building or on private property,as stated in the local tariffs.
If a building designation changes from multi to single tenancy,a PBX or switch is installed,and U S WEST Communications has
placed regulated facilities after the effective date of the tariff,U S WEST Communications owned facilities will be purchased by
the Premises Owner for a fair market price,removed and/or disabled at the Premises Owners expense.
This is to certify that the person signing below is the campus or building developer,owner,or their designee and this document
accurately reflects the intended use of this building at the time and will be relied u n by USWC in its provisio ing and any
regulatory or oceedings related this property.

STRUCTURAL SPECIFICATIONS (8-94)
1. CONDUIT: SCIJE.0 40 02 �t_
A. Entrance Conduit: No.Of Conduits Size Type
B. Distribution Conduits: No.of Conduits Size Type
C. All conduits are to be equipped with minimum 1/4"polypropylene pull line.
D. All conduit bends are to be steel sweeps or no less than PVC Schedule-80 fully encased.Long radius sweeps
are to be used In main trench sections.Standard radius sweeps may be used at closure locations.
E. All conduit runs are designed on the basis that each separate run will have no more than two(2)90°bends
without the insertion of an appropriately sized pull box.
F. Use only rigid,water-tight conduit in floor slabs.
G. No exposed cable runs which exceed 50 feet will be permitted inside of any building without the use of fitted
metallic conduit,in accordance with the National Electric Code,B.G.70-798 Section 800-50,Exception No.3.
H. All conduits will be PVC Schedule-40 or better unless specified otherwise.If steel or iron pipe is used between the
building and the property line,the pipe must be kept free from contact with any reinforcing steel or other
conductors within the building foundation wall.
Conduit(s)will be furnished,installed and maintained by the Premises Owner and reserved for the exclusive use
of U S WEST Communications
J. Conduit(s)placed in the same trench with power supply conductors/conduits must be separated by a minimum of
of 12"of compacted soil or 3'of slurry encasement and have a minimum depth of cover of not less than 24"on
private property and 36"of cover at the property line.
K. Conduits,labs,water/gas pipe,flex conduit or plumbing fittings shall not be deemed acceptable for use by
U S WEST Communications.
L. Conduit(s)terminating at a utility pole must be attached to the pole and extend to a minimum height of 12"AFG.
M. Weather heads are to be placed at a height of 20 feet AFG and must maintain a minimum clearance of 12"from
any foreign power supply source.
2. BACKBOARD:
A. No.of backboards required:

C. Backboards shall be painted with fire-retarding paint.
D. Conduit(s)shall be terminated _ .. feet above the floor and/or feet below the ceiling at
backboard location.
3. GROUNDING;
A. Premises Owner/Developer to provide a 06 AWG solid core copper ground wire from the U S WEST backboards
and connected back to the power company's multi-neutral ground source.
4 TELEPHONE SERVICE CABINETS;
A. NEMA-3R rated and equipped with full-sized hinged cover with adequate weather resistant gasket ring.
B. Inside rear panel must be equipped with 3/4"plywood backboard.
C. Cabinet to be mounted and centered at a height of 52"AFG.
D. Maintain a minimum of 12'radial clearance from any foreign power supply source/equipment.
E. Maintain a minimum of 36"of clear space to the front of cabinet for safe worker access,in accordance with the
National Electric Code,B.G.70-27,Section 110-34,Table 110-34(a).
5. SPLICE VAULTS/MAINTENANCE HOLES:
A. All splice vaults&maintenance holes are to be equipped with ladders and cable racking materials,as may be
appropriate for specified size and intended usage
B. See print for locations(s)and attachment .-for size and specifications.
1. Any items that deviate from these specifications must be cleared with the U S WEST Communications Engineer prior to the
change being made in the field by the Premises Owner/Developer.
2. Construction work on telephone supporting structure must be completed&inspected 30-days prior to the date that perma-
nent
telephone service will be required.Should Premises Owner/Developer fail to meet this time frame,U S WEST
Communications shall not be obligated to meet the tentative in-service date and shall be allowed the full 30-day period within
which to schedule and complete all of its work that may be associated with this project,commencing at the point in time that
the Premises Owner/Developer has met the requisite structural inspection.
3. FOR INITIAL/FINAL INSPECTION CALL(976 ) -45-0-25-S7 AT LEAST 48-HOURS IN ADVANCE.
4 All inside wiring must meet U S WEST Communications'minimum standard of 4-pair(8-conductor)#24 AWG color-coded
twisted pair copper wire and each must be identified by unit 11 at the telephone backboard/terminal cabinet location.
5. U S WEST Communications will require a minimum notification period of 30-days prior to wall insulation in order to schedule/
complete placement of Network Termination Wiring and SNI devices in Option-3 multi-family projects.
6. In accordance with U S WEST Communications'applicable Tariff Schedules,the Premises Owner/Developer is required to
provide the appropriate path and/or supporting structure for use by U S WEST Communications in the placement of Network
Termination Wiring,as described in items 4&5,above.The appropriate path/supporting structure may consist of,but is not
limited to,conduit(s)or pre-drilled wall studs&floor/ceiling joists.
7. Should Premises Owner/Developer fail to meet the appropriate notification schedule as stated in item 5,above,U S WEST
Communications shall not be obligated to meet the requirements until such time as Premises Owner/Developer has met the
the requisite path/structural requirements.
