RE: APPROVE TASK ORDER RENEWAL LETTER#1 FOR TUBERCULOSIS CONTROL
AND OUTREACH PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the
Tuberculosis Control and Outreach Program between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Weld
County Department of Public Health and Environment, and the Colorado Department of Public
Health and Environment, commencing July 1, 2000, and ending December 31, 2000, with
further terms and conditions being as stated in said task order renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
renewal letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Renewal Letter#1 for the Tuberculosis Control
and Outreach Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment be, arid hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 12th day of June, A.D., 2000.

State Fiscal Year 2000-01, Task Order Renewal Letter Number 1, Contract Routing Number 01-00037
Pursuant to Section F,5.of Master Contract with contract routing number 00 FAA 00008,and Task Order with contract
routing number 00 FHA 00140 and contract encumbrance number EPI0000140,hereinafter referred to as the"Original
Task Order"(a copy of which is attached hereto and by this reference incorporated herein and made a part hereof)between
the State of Colorado,DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and WELD COUNTY
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT,for the renewal term from July 1,2000,through
December 31,2000,the parties agree that the maximum amount payable by the State for the eligible services referenced is
Section C, I.,of the Original Task Order is increased by THIRTY TWO THOUSAND FIVE HUNDRED SIXTY
DOLLARS,($32,560.00)for a new total financial obligation of the State of SIXTY FOUR THOUSAND EIGHT
HUNDRED TWO DOLLARS,($64,802.00)and that Section C.2.of the Original Task Order is TWENTY FIVE
THOUSAND DOLLARS($25,900.00)for Tuberculosis Direct Observed Therapy for State Fiscal Year 2001
Statewide. The revised reimbursement schedule,which is attached hereto as"Attachment C"is incorporated herein by
this reference and made a part hereof. The first sentence in Section C,1.of the Original Task Order is hereby modified
accordingly.The first sentence of Section B.7.of the Original Task Order shall be amended to read,"The Contractor shall
provide I:he State a narrative report for the initial term of this Task Order,July 1,2000 to December 31,2000 by March I,
2001...'' All other terms and conditions of the Original Task Order are hereby reaffirmed. This amendment to the Original
Task Order is intended to be effective as of July 1,2000. However,in no event shall this amendment be deemed valid uptd
it shall have been approved by the State Controller or such assistant as he may designate.

SUBJECT: TB Control and Outreach Task Order Renewal Letter#1
DATE: June 9, 2000
Enclosed for Board review and approval is task order renewal letter#1 for the TB Control and
Outreach Program. If approved this renewal letter will become a part of the master contract
between the Colorado Department of Public Health and Environment and the Weld Cottrty
Department of Public Health and Environment (WCDPHE).
Under the provisions of this renewal letter, WCDPHE will provide active TB treatment. TB
preventive treatment, provide for and interpret chest x-rays and oilier medical evaluation
services, conduct contact investigations and TB skin test screening programs. WCDPHE gill
also follow-up with active patients to ensure they receive appropriate information and clue at it r
and they adhere to the completion of their TB therapy.
For these services, the Health Department will receive an amount not to exceed $32,560 for the
renewal term of July 1, 2000 through December 31, 2000. This is an increase of$318 over tic
previous six month's funding for the program.
