RE: APPROVE FIVE PURCHASE OF SERVICES AGREEMENTS WITH VARIOUS
PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with five Purchase of Services Agreements
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the following providers, with terms and conditions being as stated in said
agreements:
1) Ault Senior Citizens
2) Evans Senior Center
3) Kersey Senior Citizens
4) Nunn Senior Citizens
5) Tri-Town Senior Center
WHEREAS, after review, the Board deems it advisable to approve said agreements,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the five Purchase of Services Agreements between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Human Services, Area Agency on Aging, and the above listed
providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements.

SUBJECT: Purchase of Services Agreements with rural.
Weld Senior Centers
Enclosed for Board approval are five (5) Purchase of Service agreements with rural Well( uunty
Senior Centers to purchase outreach activities. These grants are in response to needs data •vhich
shows that a high percentage of Weld seniors are lonely and isolated from community lift. The
senior centers listed below have agreed to redouble their efforts to reach out to these senicn s by
implementing telephone reassurance calls, one on one visitations, buddy programs, free im al
coupons to attract new seniors to the centers and a variety of other outreach activities. (-rra its
will be given to the following:
Ault Senior Center - $1,000
Evans Senior Center - $1M00
Kersey Senior Center - $I,000
Nunn Senior Center - $1.000
Tri Town Senior Center - $1,000
Funds will be used for such items as cards, welcome baskets, free meal coupons and spec', I
events. The activities will be carried out by volunteers.

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 20th day of November,2000 by and between the Count} of Weld, St to
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Ault Senior Citizens
hereinafter referred to as "Ault."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services.
and
WHEREAS, "AAA" considers senior centers a vital link to seniors who are lonely and isolated, and
WHEREAS,"Ault" is a program providing recreational, nutritional, and educational opportunities for senior citizens.
and
WHEREAS, "Ault" proposes to implement an "Ault Outreach Program" to reach out to seniors in their community to
involve them in the social activities at the Ault Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA" hereby agrees to award grant monies to "Ault" in the sum of$1,000.
2. "Ault" agrees to mail approximately 1,000 invitations to all box holders in the Ault area. The invitation will invit
seniors to come to the center and will include a"free meal" coupon for the first senior dinner. Ault Senior Citizen
expect to recruit 20 new members to their program.
3. "Ault" agrees to document activities and results of this outreach effort. Ault Senior Citizens understand that the
grant of$1,000 may be used to pay for the costs of the outreach program and to advance the overall Ault Senior
Citizens Program.
4. "Ault" understands this is a one time only incentive award and the terms of this agreement are valid r hrough June
30, 2001.

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 15th day of November, 2000 by and between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of t :e
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the
Evans Senior Center hereinafter referred to as "Evans."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, an I
WHEREAS, "Evans" is a senior center providing recreational, nutritional, and educational opport inities for
senior citizens; and
WHEREAS, "Evans"proposes to implement a "Punch Out Loneliness Program" to reach out to a ca seniors-
NOW THEREFORE, it is mutually agreed:
1. The"AAA" hereby agrees to award grant monies to "Evans" in the sum of$1,000.
2. "Evans" agrees to recruit volunteers to conduct one-on-one contacts with area seniors to involve them in
meals, trips and other special activities at the Evans Senior Center. Approximately 200 contacts will be
made.
3. "Evans" agrees to document contacts made. Evans understands that the $1000 may be used rn any was
benefit and advance the Evans Senior Program.

WHEREAS, "AAA" considers senior centers a vital link to seniors who are lonely and isolated, and
WHEREAS, "Kersey" is a program providing recreational, nutritional, and educational opportunities Ibr senior citizc is,

WHEREAS, "Kersey" proposes to implement a"Reach Out Program"to find homebound seniors in their community
and make a difference in their lives;
NOW THEREFORE, it is mutually agreed:
I. The"AAA" hereby agrees to award grant monies to "Kersey" in the sum of$1,000.
2. "Kersey" agrees to identify homebound seniors in the Kersey area, do home visits that will include r welcome gi I.
expand a telephone calling tree, and offer volunteer respite care to families when appropriate.
3. "Kersey" agrees to document activities and results of this outreach effort. "Kersey" understands that the grant„
$1,000 may be used to pay for the costs of the outreach program and to advance the overall Kersey Senior Citize, ,
Program.
4. "Kersey" understands this is a one time only incentive award and the terms of this agreement are Valid through It lie

Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Nunn Senior Citizens
hereinafter referred to as"Nunn."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services.

WHEREAS, "AAA" considers senior centers a vital link to seniors who are lonely and isolated, and
WHEREAS, "Nunn" is a program providing recreational, nutritional, and educational opportunities for senior citizen,
and
WHEREAS. "Nunn" proposes to implement a "Building Bridges of Friendship Program"to reach out to seniors in the
Nunn and Carr areas to involve them in the social activities at the Nunn Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to the"Nunn" in the sum of$1,000.
2. "Nunn"agrees to recruit volunteers who will contact homebound seniors, new seniors to the area, to increase pho ie
calls and mailings, and to help with shopping and transportation appointments for seniors.
3. "Nunn"agrees to document activities and results of this outreach effort. Nunn Senior Citizens understand that it
grant of$1,000 may be used to pay for the costs of the outreach program and to advance the overall 'Junn Senior
Citizens Program.
4. "Nunn" understands this is a one time only incentive award and the terms of this agreement are valid through Jun
30, 2001.

Tri-Town Senior Center hereinafter referred to as "Tri-Town."

services, and
WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, at .1
WHEREAS, "Tri-Town" is a senior center providing recreational, nutritional, and educational op=portunities t
senior citizens; and
WHEREAS, "Tri-Town" proposes to implement a"Punch Out Loneliness Program"to reach out to area
seniors;
NOW THEREFORE, it is mutually agreed:
1. The"AAA" hereby agrees to award grant monies to "Tri-Town" in the sum of$1,000.
2. "Tri-Town" agrees to send once a month "Thinking of You " cards and free meal coupons to area senior to
encourage their participation at the senior center. Volunteers will write the cards.
3. "Tri-Town" agrees to document contacts made. "Tri-Town" understands that the $1000 ma: he used in
any way to benefit and advance the Tri-town Senior Program.
