RE: APPROVE CHANGE ORDER LETTER FOR INTENSIVE SUPERVISION PROGRAM
(ISP) NON-RESIDENTIAL SERVICES AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Change Order Letter for the Intensive
Supervision Program (ISP) Non-Residential Services between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Community Corrections Board, and the Department of Corrections, Division of
Adult Parole Supervision, commencing upon full execution of said change order letter, and
ending June 30, 2000, with further terms and conditions being as stated in said change order
letter, and
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter for Intensive Supervision Program (ISP)
Non-Residential Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Community
Corrections l3oard, and the Department of Corrections, Division of Adult Parole Supervision, be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 15th day of May, A.D., 2000.

SLBJECT: Change Order Letter
As the end of the fiscal year approaches adjustments are being made to the funding for ISP Non-
Residential Services.
Enclosed you will find a change order letter, which modifies the dollar allocation from the
original contract, between the Department of Corrections, Division of Adult Parole and your
agency. Please read, sign and return before May 22, 2000.

CHANGE ORDER LETTER
Board of Commissioners of Weld County
On behalf of the Weld County Community Corrections Board

Contractor:
Pursuant to paragraph 2.B.(2) of the Contract between the Department of Corrections, Division
of Adult Parole Supervision and Board of Commissioners of Weld County on behalf of the Weld
County Community Corrections Board,the Division of Adult Parole Supervision hereby notifies
you that the maximum amount payable from the State of Colorado ISP Non-Residential Service,
under Contract.Number 00CAA00175, covering the period July 1, 1999 through June 30, 2000
is hereby decreased by $4,000.00 to a new total of ($19,524.00). The rate remains unchanged.
Paragraph 2.B. (2)of Contract 00CAA00175 shall now read:
During the period of this Contract,upon receipt of proper billing from the Contractor as provided
in contract paragraph 1.5 the State will pay an amount not to exceed Nineteen Thousand Five
Hundred Twenty Four Dollars ($19,524.00) for ISP services herein described for the daih
average rate of approximately two hundred thirty one (231) Department of Corrections parolees.
This amendment to the Contract is intended to be effective May 1, 2000,but in no event shall this
amendment be deemed valid until the State Controller or his designee has approved it.
Please sign, date and return all copies of this Letter on or before May 21, 2000.
