RESOLUTION
RE: APPROVE TASK ORDER FOR PROVISION OF STD MEDICATIONS AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order for the Provision of STD
Medications between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing July 3, 2000, and ending June 30, 2005, with further terms and conditions being
as stated in said task order, and
WHEREAS, after review, the Board deems it advisable to approve said task order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order for the Provision of STD Medications between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of July, A.D., 2000, nunc pro tunc July 3, 2000.

TASK ORDER
This TASK ORDER is made this 20th day of June,2000, by and between: the State of Colorado, for the use and benefit of the
COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, whose address or principal place of
business is 4300 Cherry Creek Drive South,Denver,Colorado 80246,hereinafter referred to as"the State";and,the
BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY (a political subdivision of the state of Colorado),
whose address or principal place of business is 915 10th Street.3rd Floor.Greeley,Colorado 80631, for the use and benefit
of the Weld County Department of Public Health and Environment,whose address or principal place of business is 1555
North 17th Avenue,Greeley,Colorado 80631,hereinafter referred to as"the Contractor".
FACTUAL RECITALS
Section 29-1-201,9 C.R.S. as amended,encourages governments to make the most efficient and effective use of their powers
and responsibilities by cooperating and contracting with each other to the fullest extent possible to provide any function,
service, or facility lawfully authorized to each of the cooperating or contracting entities,and to this end all State contracts with
its political subdivisions are exempt from the State's personnel rules and the State procurement code. The State has tbnmulatee'
a comprehensive State plan,with associated budgets,relative to the State's programs and services which allocates funds to
local health agencies in order to provide certain purchased services to the people of Colorado. This funding is to be allocated
through task order contracts with local health agencies. The State, in order:to carry out its lawful powers,duties, and
responsibilities under Section 25-4-401,ft Seq., 8 C.R.S.,as amended;and,to effectively utilize legislative appropriations
made and provided therefore, in coordination with like powers,duties, and responsibilities of the Contractor, has determined
that public health services are desirable in Weld County ,Colorado.
Pursuant to 24-4-401 C.R.S., et seq, it is the duty of the State and local health departments to investigate, control, and treat
contagious sexually transmitted diseases. The State has received funding from the Department of Health and Human Services,
Center for Disease Control and Prevention(CDC)for the treatment of sexually transmitted diseases,pursuant to 93.9910 of the
Catalog of Federal Domestic Assistance(CFDA).
Authority exists in the Law and under Master Contract routing Number 00 FHA 00008.
NOW THEREFORE, in consideration of their mutual promises to each other, stated below,the parties hereto agree as
follows:
A. PERIOD OF PERFORMANCE AND CONTRACT TERMINATION. The effective date of this [ask Order is
July 3,2000. The term of this Task Order shall commence on July 3.2000,and continue through and including
June 30.2005, unless sooner terminated by the parties pursuant to the terms and conditions of the Master Contract.

B. SCOPE OF WORK.
I. For Sexually Transmitted diseases that are reportable according to the current regulations of the Collorado
Board of Health,the State shall provide drugs and biological supplies for diagnosis and treatment.
2. Drugs and supplies provided shall be used by the Contractor under the supervision of a licensed physician
and in accordance with the most recent edition of the Sexually Transmitted Disease Treatment Guidelines
published by the Centers for Disease Control and Prevention.
3. The Contractor shall provide to the State documentation of authorization to receive prescript on drugs on or
before July 17, 2000.
4. The Contractor shall provide to the State annual census reports that document the number of cases of
sexually transmitted diseases treated with each of the drugs provided. The report shall include totals of
diagnoses for each disease, whether the diagnosis was derived from a positive laboratory test or presumptiv •
based on symptoms,and differentiate between actual cases and individuals treated as contacts. For each
calendar year,the census report shall be due to the State on or before March 31"of the following year and
submitted to the attention of: Rebecca Jordan,Colorado Department of Public Health& Environment,

SUBJECT: Task Order for the Provision of STD Medications
DATE: July 24, 2000
Enclosed for Board review arid approval is a task order which, if approved, will become pill of
the Master Contract between Weld County and the Colorado Department of Public Health and
Environment (CDPHE) for the provision of STD (Sexually Transmitted Disease) medications.
In the past, CDPHE supplied treatment medications for STD directly to local health Departments.
To increase program efficiency and ensure compliance with State regulations, the State hus
arranged for the University of Colorado Health Sciences Center School of Pharmacy to distribute
the drugs from the Apothecary Pharmacy in Boulder. This task order outlines the responsibilities
of Weld County Department of Public Health and Environment and of the State Health
Department for the receiving and distribution of these medications As CDPHE has received
funding from the Department of Health and Human Services Center for Disease Control and
Prevention (CDC) for the treatment of sexually transmitted diseases, there is no charge to Weld
County for these medications.
