RE: APPROVE CONTRACT AMENDMENT AND AUTHORIZE CHAIR TO SIGN -THE
VILLA AT GREELEY, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Amendment between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Community Corrections Board, and The Villa At
Greeley, Inc., with terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract Amendment between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Community Corrections Board, and The Villa At Greeley, Inc., be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is., authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 10th day of April, A.D., 2000.

WELD COUNTY COMMUNITY CORRECTIONS
AMENDMENT TO CONTRACT WITH
THE VILLA AT GREELEY, INC.
THIS AMENDMENT TO CONTRACT, made this 10 day of April , 2000, by and
between the Board of Commissioners of Weld County, on behalf of Weld County Community
Corrections Board, P.O. Box 758, Greeley, CO 80632, hereinafter referred to as "the Board," and
The Villa At Greeley, Inc., 1750 6th Avenue, Greeley, CO 80631, hereinafter referred to as The
Contractor."
WITNESSETH:
WHEREAS, the parties hereto entered into a contract with The Villa At Greeley, Inc., dated
July 1, 1999, for the provision of community corrections services; and
WHEREAS, the parties now desire to amend said contract to reflect ongoing practices
wherein the Contractor arranges for the provision of appropriate electronic monitoring of approved
offenders by an independent contractor and the Board then pays invoices submitted by such
independent contractor for the provision of such services.
NOW, THEREFORE, the parties hereto agree to amend their Contract of July 1, 1999, as
follows:
4. SPECIAL PROVISIONS:
F. Electronic Monitoring. Contractor shall arrange for appropriate electronic monitoring of
approved offenders by an independent contractor. Invoices for such electronic monitoring
shall be submitted by such independent contractor to the Board and the Board agrees to
pay the same within 30 days of receipt.
IN WITNESS WHEREOF, the parties hereto have executed this Contract on the day first above
written.
