RE: APPROVE TASK ORDER RENEWAL LETTER#1 FOR FAMILY PLANNING
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the
Family Planning Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2000, and ending June 30, 2001, with further terms and
conditions being as stated in said task order renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
renewal letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Renewal Letter#1 for the Family Planning
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 12th day of June, A.D., 2000.

Enclosed for Board review and approval is task order renewal letter#1 for the Family Plan in
Program. If approved this renewal letter will become a part of the master contract betwe:n thz
Colorado Department of Public Health and Environment and the Weld County Departmeiii : f
Public Health and Environment (WCDPHE).
Under the terms of this renewal letter, WCDPHE will be reimbursed an amount not to ex:ccd
$96,061 for the time period July 1, 2000 through June 30, 2001 to provide contraceptive servi v s
to both men and women in their reproductive years. These services will include education an('
counseling, comprehensive health and social histories, physical examinations, supplies, fi( ,lov
up and/or referral services. This funding is an increase of$2,541 over the previous year's
funding for this program.
I recommend your approval of this renewal letter.

State Fiscal Year 2000 - 2001, Task Order Renewal Letter Number 1 , Contract Routing Number 0.0-0X066
Pursuant to paragraph C4 of the Task Order with contract routing number 00-00971 and contract encumbrance number
FPP00-00971, hereinafter referred to as the"Original Task Order" (a copy of which is attached hereto and by this reference
incorporated herein and made a part hereof) between the State of Colorado, Department of Public Health and Environment
and BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY , for the renewal term from July I, 2000 through
June 30, 2001, the parties agree that the maximum amount payable by the State for the eligible services referen:.ed in
paragraph Cl of the Original Task Order is increased by NINETY SIX THOUSAND AND SIXTY ONE Dollars,
($96,061.00) for a new total financial obligation of the State of ONE HUNDRED TWENTY NINE THOUSAND FIVE
HUNDRED EIGHTY EIGHT DOLLARS, ($129588.00 )to provide family planning services to 1574 clients and n!r
cryotherapy services. For the months of July through December inclusive,the Contractor shall bill for I/6th of the 5$J 916.00
federal Title X contraceptive services amount(S 9 652.67/month). For the months of January through June inc lusivc, he
Contractor shall bill I/6th of the 5 36,195.00 State general fund contraceptive services amount ($6 032.00/month). he
cryotherapy services at$75.00 per client and/or pap smear screenings may be billed monthly as expended up to 31,9 Si:.00 and
will be reimbursed from state general funds.
The first sentence in paragraph I of the Original Task Order is hereby modified accordingly. All other terms and condaions of
the Original Task Order are hereby reaffirmed. This amendment to the Original Task Order is intended to be effective as of
July 1, 2000 . However, in no event shall this amendment be deemed valid until it shall have been approved ba the State
Controller or such assistant as he may designate_ Please sign, date, and return all 4 originals of this Task Order Renewal Le'zer
as soon as possible to the attention of: Candace Grosz, Fiscal Officer, Women's Health Section,CDPHE,4300 Chevy Creek
Drive South, 4'5 Floor, Denver, Colorado 80246-1530. One original of this Task Order Renewal Letter will be returned to you
when fully approved.
