RE: APPROVE CHANGE ORDER LETTER FOR AREA AGENCY ON AGING AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Change Order Letter for the Area
Agency on Aging between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging, and the Colorado Department of Health Care Policy and Financing,
commencing May 18, 2000, and ending June 30, 2000, with further terms and conditions 7eing
as stated in said Change Order Letter, and
WHEREAS, after review, the Board deems it advisable to approve said Change Order
Letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter for the Area Agency on Aging between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and the
Colorado Department of Health Care Policy and Financing be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said Change Order Letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of April, A.D., 2000.

Enclosed for board approval is an amendment to the contract between Weld County Board of
Commissioners and through the Weld County Division of Human Services Area Agency ]n
Aging. The maximum contract reimbursement will be changed from $384,642 to $367 832
This change is based on the number of clients that were served in the third quarter of this
contact year (439).

In accordance with Paragraph 10 of Page 2 of Exhibit A of contract routing number 3300-1324 between :he State of
Colorado Department of Health Care Policy and Financing and Weld County Board of Commissioners b4 and
through the Weld County division of Human Services" Area Agency on Aging covering the period of Ju y I, 1999
through June 30, 2000 the undersigned agree the first sentence of Paragraph 4, Page 5, Price/Cost is her amended
to read:
The State shall reimburse the contractor's reasonable, allowable costs, as defined herein, not
exceeding $367,882 for work associated with the Scope of Work described under paragraph 2(a•
above.
This constitutes a (n) decrease in the maximum compensation to be made to the Contractor by the State
under this contract of$16,760.
The sentences on Page 2 of Exhibit A to the contract which now read:
However, total compensation for the contract term shall not be increased under this mechanism )y
more than $36,034 above the base contract amount, i.e., to a maximum total compensation or
$384,642 The maximum increase above the base contract amount represents 43 clients above th•
base client figure; the figure of 459 shall be known as the "maximum reimbursable client figure•.
are amended to read:

$367,882. The maximum increase above the base contract amount represents 23 clients above tie

This amendment to the contract is intended to be effective as of May 18, 2000, but in no event shall it be deemed
valid until it shall have been approved by the State Controller or such assistant as he may
designate.
