RE: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT AND
AUTHORIZE CHAIR TO SIGN - RVNA HOME HEALTH CARE, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Aging, and the RVNA Home Health Care, Inc., commencing August 1, 2000, and
ending December 31, 2000, with further terms and conditions being as stated in said
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement Amendment between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and the
RVNA Home Health Care, Inc. be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of August, A.D., 2000, nunc pro tunc August 1, 2000

SUBJECT: Purchase of Service Agreements with RVNIA
Home Health Care, Inc.
Enclosed for Board approval are three (3) Purchase of Services Agreements with RVNA 1 Iom
Health Care, In. to provide in home health care and respite care services.
1. Purchase of Services Agreement Amendment between the Area Agency on Aging ,ind
RVNA amending the original agreement of January 2, 2000. The amendment is to
increase the hourly rate for home health care from $10.65 per hour to $10.85 per hour.
2. Purchase of Services Agreement between the Area Agency on Aging and RVNA to
purchase 1,125 hours of in-home respite care for 16 older adults and their families 'NH
agreement will extend from August 1, 2000 to June 30, 2001. This agreement wil l be
funded from new State General Fund monies.
3. Purchase of Services Agreement between the Area Agency on Aging and RVNA 6 r
1,575 hours of home health care for 25 60 plus older. This agreement will extend Iron
August 1, 2000 to June 30, 2001. This agreement will be funded from new State (ienei al
Fund monies.
PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of August, 2000 by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA" and RVNA Home Care Services, Inc., hereinafter referred to as "RVNA."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home health care services from
"RVNA", and
WHEREAS, the "AAA" and "RVNA" are desirous of amending the original agreement of January
2, 2000,
NOW THEREFORE, it is mutually agreed:
I. The "AAA" will reimburse "RVNA" $21,175 for 2,000 units of home health care for 55
persons at $10.85 per unit of service.
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. The terms of this Amendment extend from August 1, 2000 through December 31, 2000.
This Amendment will be terminated immediately in the event of loss of funding.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the day first
written above.
