Speckman, Executive Director, Human Resources tt Akk subject: Agreement with School District RE-5J Enclosed for Board approval is an Agreement between Head Start and School District RE-5J. The School District will be providing classroom space for the Head Start Program at a cost of $150.00 per month at the Letford Trailer site in Johnstown. The School District will be providing bus transportation on its regular route at no charge, those children who are not on the regular route will be provided transportation at a charge of $8.00 per month per child . The School District will also provide hot lunches for the Head Start Program at a cost of $1.25 per child and $1.30 per adult. The term of this Agreement shall be from September 10, 1984 through May 18, 1985. If you have any questions, please feel free to telephone me at 353-0540. 8404)27 f/ y ' (1 '7,11 AGREEMENT THIS AGREEMENT, made and entered into this 13th day of August , 1984, by and between the Weld County Division of Human Resources, Head Start Program, hereinafter referred to as "Human Resources," and Weld County School District RE-5J, hereinafter referred to as "School District. " WITNESSETH: That for and in consideration of the covenants and agreements herein agreed to be kept and performed, School District hereby leases and agrees to provide unto Human Resources the following described premises and services upon the following terms and conditions: 1. School District agrees to provide classroom space for a rental of $150.00 per month at the Letford Trailer Site, Johnstown, Colorado, for one (1) Head Start session, beginning September 10, 1984, and ending May 18, 1985, contingent upon School District's state requirements. Billing for rent shall be made monthly along with the school lunch billing noted below. 2. School District agrees to provide bus transportation for Head Start children on its regular route within the district at no charge. Those children not on the regular route will be provided transportation at a charge of $8.00 per child per month not to exceed the total of $450.00 during the year. Billing shall be made monthly along with school lunch billing noted below. 3. School District agrees to provide hot lunches for the Head Start classroom at a price of S1. 25 per child and $1.30 per adult. The School District will submit a bill for such services to Human Resources by the 10th of the month for the preceding month. 4. The School District agrees to provide meals in accordance to the regulations of the United States Department of Agriculture Child Care Food Program as shown in Attachment A of this Agreement. 5. Human :- 'sources will notify the School strict within the first hour of the beginning of the school day if there is a variance in the number of meals needed for that day. Human Resources shall notify the School District the day before holidays and vacations that meals will not be needed for those specified day(s) . 6. Thorough and complete custodial care shall be provided by the School District daily within the rental agreement specified above. 7. Human Resources agrees to provide telephone service for the Head Start staff for any telephone installed in the Letford Trailer. 8. Human Resources agrees to pay the hereinabove mentioned agreed price for all meals served to Head Start children and staff, and such reimbursement will be made within three (3) weeks of the receipt of a statement of such charges from School District. 9. Human Resources agrees that it will throughout the term of the Agreement comply with the rules and regulations of School District in effect from time to time as they may affect Head Start children. 10. It is mutually agreed that this Agreement may be cancelled by either party giving the other party at least 30 days written notice. This Agreement wiT1 terminate immediately in the event of a loss of funding to the program. IN WITNESS WHEREOF, the parties hereunto have caused this Agreement to be duly executed as of the day and year first hereinabove set forth. Speckman c ` o- n�,e "(- Exe hive Director D,pu y Ccwr.t7 C rk Lunch or Supper ( - 'Attachment A The minimum amounts of food components to be served as lunch or supper as set forth are as follows: Food Components Age 3 up to 6 MILK Milk, fluid 3/4 cups VEGETABLES AND FRUITS • Vegetable(s) and/or fruit(s)2 1/2 cup total - BREAD AND BREAD ALTERNATIVES3 Bread 1/2 slice or Cornbread, biscuits, rolls, muffins, etc.4 1/2 serving or Cooked pasta or noodle products 1/4 cup Cooked cereal grains or an equivalent quantity of any combination of bread/bread alternate. 1/4 cup MEAT AND MEAT ALTERNATIVES Lean meat or poultry or fish5 1 1/2 oz. or Cheese 1 1/2 oz. or Eggs l egg or Cooked dry beans or peas 3/8 cup or Peanut butter or an equivalent of any combination of meat/meat alternative. 3 tbsp. 1. For purposes of the requirements outlined in this subsection, a cup means a standard measuring cup. 2. Serve 2 or more kinds of vegetable(s) and/or fruit(s). Full-strength vegetable or fruit juice nay be counted to meet not more than one-half of this requirement. 3. Bread, pasta or noodle products, and cereal grains shall be whole grain enriched; cornbread, biscuits, rolls, muffins, etc. , shall be made with whole grain or enriched meal or flour. 4. Serving sizes and equivalents to be published in guidance materials by FNS. 5. Edible portion as served. Hello
