RE: APPROVE GRANT AGREEMENT/CONTRACT FOR ADULT NUTRITION AND
EXERCISE PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Agreement/Contract for the
Adult Nutrition and Exercise Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment, commencing October 15, 2000, and ending October 14, 2001, with further
terms and conditions being as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Grant Agreement/Contract for the Adult Nutrition and Exercise
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted

Enclosed for Board review and approval is a grant agreement/contract between Weld County
Department of Public Health and Environment (WCDPHE) and Colorado Action for Healthy
People for renewal funding for an adult nutrition and exercise program.
This agreement will provide funding for nutrition classes at senior nutrition sites and other
locations within the County. During this agreement period, Health Department staff members
will identify another new location for introductory adult exercise classes. They will also continue
to coordinate the Weld County Diabetes Coalition and provide a -.cries of three classes on Din ine
With Diabetes. To accomplish these efforts,WCDPHE will be collaborating with other agent it s
such as Weld County Area Agency on Aging, Diabetes Disease Management of North ('olora)P(
Medical Center, CSU Extension Services and Aims Community College.
For these services, WCDPHE has been awarded the sum of$10,000 for the time period t)ctohet

GRANT AGREEMENT/CONTRACT
Grant Number: HHAB2-00TR-116
Weld County Dept. of Public Health and Environment, (hereinafter "grantee"), a
nonprofit or government organization, hereby accepts the awarded grant amount of $10,000
from Colorado Action for Healthy People (hereinafter"CAHP") to conduct a healthy habits
project. The term of this award is from October 15, 2000 through October 14, 2001. Final
financial and progress reports will be due on October 31, 2001.
The grantee agrees to administer the grant in compliance with the proposal submitted to
CAHP (which is hereby incorporated as part of this contract as "Attachment A") and the
following provisions:
1. Use of Grant Funds: This grant is restricted to the purpose specified in Attachment A and is
to be spent in accordance with Grantee's proposed budget. Any part of the grant funds not so
used within the term of the grant must be returned to CAHP within 30 days after the
completion of the project or upon expiration of the grant.
2. Local Support: Grantee agrees to provide local cash and/or in-kind support in the amount
indicated in the budget section of Attachment A
3. Cost Principles:
a. Grant funds will be used only for project purposes set forth in the original application
submitted to CAHP, unless changes are subsequently approved by CAHP, as
explained in #5 below.
b. Grant funds must be obligated during the grant period. No grant funds may he
applied to any project expenses incurred prior to the date of this agreement..
4. Grant Accounting and Records:
a. Grantee agrees to maintain adequate accounting records and to obtain documentation
for all grant expenditures.
b. Grantee agrees to document all local support. Grantee agrees to provide other reports
and meet all other financial requirements as established by CAHP.
c. Grantee agrees to maintain records and accounts consistent with generally accepted
accounting principles, and to provide for such fiscal control as is necessary to assure
proper disbursing of, and accounting for, project grant funds.

d. The Grantee certifies that accounts and supporting documentation relating to project
expenditures will be adequate to permit an accurate and expeditious audit. An audit
may be made at any time by CARP, its designated representatives, or any appropriate
agency of the United States government.
5. Project Changes: Any significant change in project activities or expenses must receive prior
written approval from CAHP. These changes include but are not limited to changes in the
project's scope, purpose or activities, changes in expenses of more than 20 percent of an
approved budget category, and changes in the duration of the grant period.
6. Payment of Grant Funds: Awards of CARP funds will be made in the following manner,
unless otherwise negotiated with CARP:
a. One half(50%)of the total grant shall be paid to grantee by October 31, 2000.
contingent upon receipt of the signed grant agreement, and approved reconciliation
of all CAHP funds awarded in a prior grant year.
b. The second payment shall be made in an amount up to an additional 25 percent of the
total grant upon receipt of a signed reimbursement statement and a six-month
progress report (formats to be provided by CAHP). The reimbursement statement
shall include dates, amounts, brief description of the expenditures and the total
requested. Original receipts or other supporting documentation shall be kept by the
Contractor.
c. The remainder of the total grant (25 percent) shall be paid upon receipt and approval
of the final reimbursement request and the final progress report.
d. All progress reports and requests for reimbursement should be mailed, in duplicate,
to:

7. Indemnification: Grantee agrees to indemnify and hold harmless Colorado Action for
Healthy People from any and all actions, claims, demands, costs or judgments arising out of
or in any way connected with Grantee's use or expenditure of the monies paid to Grantee
under this Agreement. This section shall not apply to state, county, city, town, township, or
school district grantees.
8. Termination: In the event that the Grantee fails to meet the terms of this contract or if n the
judgment of CAHP the Grantee becomes unable to carry out the purposes of the grant, CAHP
may terminate this Agreement at any time prior to the expiration of the grant. If this
Agreement is so terminated, Grantee shall, within 30 days of the effective date of
termination, repay to CAHP all unexpended grant funds. Grantee shall provide to CAHP a
full and complete accounting of all receipts and disbursements of funds and expenditures
incurred under this Agreement through the date of repayment to CARP.

9. General Provisions: This grant is a regrant of funds made available by The Colorado Trust to
CAHP.
10. Acknowledgment: The Grantee will acknowledge CAHP's funding on any material
developed through this grant.

Continuation Funding Application
During 2000-2001 we plan to build on the collaborative foundation that we have created
this year. We have worked with several Senior Centers that have congregate meals, and plat tc
expand these nutrition classes to other seniors. This year we have completed the series of our
classes at two sites, and have two additional sites scheduled for early fall. During the 2000-2001
year we would complete the series of four classes at three additional Senior Centers. As another
way to provide nutrition education, we also plan to move the kiosk with the touch screen
computer monitor and the La Cocina Saludable computer program to Senior Centers throughout
the county.
We have also worked very hard at making inroads into the Jesus Rodarte Cultural Center.
a facility used primarily by the Hispanic population. We have started offering one class a month
on a nutrition/ health topic to a group of seniors on Friday. and plan to continue these during, the
next year if the continuation funding is received.
As noted in the Six-month Progress Report, the walking program has been a complete.
bust. In its place, we are using grant funds to provide scholarships for the initial senior exercise
classes coordinated by Aims Community College at Hill 'N Park Senior Center in Greeley, and
Governor's Farm in Windsor. We hope that once residents at these locations are "addicted' to
the exercise classes, they will be willing to pay the fee, or fill out the paperwork to qualify for lee
waivers. These will start in September. In the continuation year of funding, we will again work
with the Aims Community College Senior Program coordinator to identify another new location
for introductory adult exercise classes. One possible site is Ft. Lupton which is in the process of
constructing a new Community Center.
We will also use the funding to continue the collaboration of the Weld County Diabetes
Coalition. This has included outreach in the community through Health Fairs, response to
individual requests, and monthly meetings to share information and coordinate activities CSIJ
Extension employees are active members of the Coalition. This year we had a presentation≤
about the diabetes information classes held in eastern Colorado and the Dining with Diabetes
curriculum. Currently the CSI I FSNLP staff member is planning to pilot the Dining with
Diabetes classes at Sunrise Community Health Center this fall. Because of FSNFP funding
guidelines, these classes must be limited to low income individuals If the Nutrition and Exercise
Grant funding is received for the second year, the project coordinators, both registered dietitians,
will present the Dining With Diabetes classes to the general public. This would be in
cooperation with the Diabetes Disease Management Program (formerly Wellness Services) at
North Colorado Medical Center. Initial discussion is to offer the series of three classes in
November, 2000 during National Diabetes Month, and again in March, 2001, National Screening
for Diabetes.
After learning about Operation Frontline at the grantee's meeting in June, we returned to
Greeley and were delighted to learn that CSU Extension staff members were already at work to
bring the program to Greeley. We are helping recruit participants for the first series of classes in
August. Since Operation Frontline is fully funded by Share Our Strength, the Nutrition and
Exercise grant would assist by supplying child care during the classes.

These agencies will continue as collaborators in the Nutrition and Fitness Grant:

There has been some reorganization at North Colorado Medical Center, and we will he
collaborating during the continuation of the grant with a different section than we attempted to
work with last year for the walking program.

Also included with the letter of collaboration from the Diabetes Disease Management team, is an
unsolicited email from the CSC Extension which clearly expresses the strengths of the
collaborative efforts in Weld County.

diabetes, arthritis, and/or chronic obstructive pulmonary disease, through increased exercise and better
nutrition in the adult population in Weld County.

Budget Narrative.
Please explain how the finds requested for each budget item will be used. Be as specific
as possible. Provide detail for local cash and in-kind.
Project Director and Other Personnel This includes four hours a week for Marian Ruge and
three hours a week for Olivia Martinez, a bilingual Health Educator in our department. She has
been working with the classes at Jesus Rodarte Center during the last few months of the grant foi.
1999-2000. Because we have added the extra staff person, we have incorporated in-kind time for
our county-funded positions to support the grant. Five hours a week of time for the grant from
Gaye Akin are local in-kind. as well.
Staff Time from Collaborating Agencies This program relies on collaborating with several
agencies within Weld County. As you have noted in the Objectives, we rely on actions by many
persons who are not funded by this grant. Therefore, we have indicated $10,000 local in-kind for
their contributions.
Printing/Duplicating Although half of the printing is provided as in-kind, we also use the County
Print Shop and Kinko's for larger print orders, and these will be billed to the grant. Also.. any
printed materials, such as cookbooks, that are purchased as educational materials are from this
line item..
Telephone, Postage, and Supplies These are all provided as local in-kind.
Informational/Educational Materials These include both the materials used as part of the teaching
materials for the classes, such as foods high in calcium when teaching a class about osteoporosis
prevention, as well as incentives, such as passes to the warm water therapy pool, which
encourage exercise. This is decreased considerably from last year's budget because we found
that we had overestimated this cost.
Travel We target the Senior Centers outside Greeley for the classes, so reimbursement for travel
is part of the budget.
Meeting Expenses This is the support for the Weld County Diabetes Coalition.
Other: Scholarships for Adult Fitness Classes and Child Care As mentioned in the narrative, the
grant would provide scholarships for the initial adult exercise classes offered by the Aims
Community College at the new Community Center in Fort Lupton. By using scholarships this
year we have been able to start exercise classes at two other locations in Weld County. In-kind
includes the vouchers provided for the second quarter of classes by Aims.
Learning is optimized when there are not a lot of distractions. Having safe and engaging child
care during the Operation Frontline classes will increase the effectiveness of the teaching.
Equipment and Space Rental There is not a need for additional equipment for this grant, and we
are fortunate to have a new building with adequate meeting space. Also, collaborating agencies
such as North Colorado Medical Center, the Senior Centers and Jesus Rodarte Cultural Center
provide space for classes without charge.

We are writing in regards to our affiliation with the Weld County Diabetes
Coalition. The Diabetes Disease Management team at North Colorado Medical
Center in Greeley has been part of the coalition since its establishment.
Over the years, we have worked with this group in many areas, including health
fairs, screenings, community awareness, education materials, support groups,
and networking with other services offered throughout Weld County. We
commend their leadership, support and development of new services.
We anticipate working with the coalition to offer our patients the "Dining with
Diabetes" series during November (National Diabetes Month) and again in
March (National Screening for Diabetes Day). We encourage your continued
support of the Weld County Diabetes Coalition.
