RE: APPROVE TASK ORDER CHANGE LETTER#1 FOR IMMUNIZATION PROGRAM
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Change Letter#1 for the
Immunization Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing upon full execution of said change letter, and ending December 31,
2000, with further terms and conditions being as stated in said change letter, and
WHEREAS, after review, the Board deems it advisable to approve said change letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW., THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Change Letter#1 for the Immunization Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 1st day of March, A.D., 2000.

SUBJECT: Change Order Letter#1 to the Immunization Program Task Order
DATE: February 28, 2000
Enclosed for Board review and approval is Change Order Letter Number 1 to the Immunization
Program Task Order between the Colorado Department of Public Health and Environment and
Weld County.
This change order letter is being issued to reduce the amount of funding for this program by 20%
or $5,400 for the time period January 3, 2000 through December 31, 2000. When the State
Health Department issued the original task order in early January they were anticipating level
funding from the Centers for Disease Control for this program. However, the State's funding
was actually reduced, and they are in turn reducing the funding for local contracts by 20%.
Weld County Department of Public Health and Environment will accommodate this cut in
funding by reducing the scope of work for this contract period. With the reduction, the total
funding provided will now be $21,600.
Health Department staff recommends your approval of this change order letter.

State Fiscal Year 2000-01, Task Order Change Order Letter Number 1, Contract Routing Number 00-00193
Pursuant to paragraph 5 of Section F of Master Contract with contract routing number 00 FAA 00008, and Task Order
with contract routing number 00 FHA 00133 and contract encumbrance number EP10000133, hereinafter referred to as
the"Original Task Order"(a copy of which is attached hereto and by this reference incorporated herein and made a part
hereof)between the State of Colorado, Department of Public Health and Environment and WELD COUNTY
HEALTH DEPARTMENT for the renewal term from January 3,2000,through December 31,2000,the parties agree
that the maximum amount payable by the State for the eligible services referenced in paragraph 1 of section C of the
Original Task Order is decreased by FIVE THOUSAND FOUR HUNDRED DOLLARS,($5,400.00) for a new total
financial obligation of the State of TWENTY ONE THOUSAND SIX HUNDRED DOLLARS, ($21,600.00). The
first sentence in paragraph 1 of section C of the Original Task Order is hereby modified accordingly. All other terms
and conditions of the Original Task Order are hereby reaffirmed. This amendment to the Original Task Order is
intended to be effective as of March 1,2000. However, in no event shall this amendment be deemed valid until it shall
have been approved by the State Controller or such assistant as he may designate.
