The undersigned is an authorized representative of the entity identified below.
We (the undersigned) are aware of and understand the mining and reclamation plans for the
above-named subject location and have spoken with a representative of L.G. Everist regarding
the proposed plans. We have no problem with the mining and reclamation plans being
proposed. We have been further assured that if any damage occurs to any of our structures
which are located within 200 feet of the mining limit that L.G. Everist will repair any damage at
its expense or provide full compensation for any damage.
The signatures of the L.G. Everist, Inc. representative and the undersigned below confirm such
agreement and repair/compensation obligation between the parties.
