HomeMy WebLinkAbout20003210.tiff RESOLUTION RE: APPROVE FOUR PURCHASE OF SERVICES AGREEMENTS WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with four Purchase of Services Agreements between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and the following service providers, with terms and conditions being as stated in said agreements: 1) Milliken Senior Center 2) Erie Senior Citizens 3) Fort Lupton Senior Center 4) Hill-N-Park Senior Center WHEREAS, after review, the Board deems it advisable to approve said agreements, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the four Purchase of Services Agreements between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and the abovelisted providers be, and hereby are, approved. Services COLORADO DATE: December 18, 2000 SUBJECT: Purchase of Services Agreements with rural Weld Senior Centers Enclosed for Board approval are four(4) Purchase of Service agreements with rural Weld County Senior Centers to purchase outreach activities. These grants are in response to needs data which shows that a high percentage of Weld seniors are lonely and isolated from community life. The senior centers listed below have agreed to redouble their efforts to reach out to these seniors by implementing telephone reassurance calls, one on one visitations, buddy programs, free meal coupons to attract new seniors to the centers and a variety of other outreach activities. Grants will be given to the following: Milliken Senior Center- $1,000 Erie Senior Center- $1,000 Fort Lupton Senior Center- $1,000 Hill N Park Senior Center - $1,000 Funds will be used for such items as cards, welcome baskets, free meal coupons and special events. The activities will be carried out by volunteers. 2000-3210 Purchase of Services Agreement THIS AGREEMENT, made and entered into this 15th day of December, 2000 by and between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Milliken Senior Center hereinafter referred to as "Milliken." WITNESSETH: WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services, and WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and WHEREAS, "Milliken" is a senior center providing recreational, nutritional, and educational opportunities for senior citizens; and WHEREAS, "Milliken" proposes to implement a"WWW.Com" or Who, Where, and Why in Our Community Program to reach out to area seniors; NOW THEREFORE, it is mutually agreed: 1. The "AAA" hereby agrees to award grant monies to "Milliken" in the sum of$1,000. 2. "Milliken" agrees to recruit volunteers to make one on one contact with any senior person who is referred by anyone in the community. The visitation will include an introduction to the Milliken Senior Program, a welcome gift and invitation to a free meal at the senior center. 3. "Milliken" agrees to document contacts made. "Milliken"understands that the $1000 may be used in any way to benefit and advance the Milliken Senior Program. The "AAA"hereby agrees to award grant monies to"Hill N Park" in the sum of$1,000. 2. "Hill N Park" agrees to implement a weekly project of reaching out to five(5) senior citizens per week, host meals for new persons, provide friendship baskets for Thanksgiving and Christmas, and provide free coupons to encourage first time participants to the senior nutrition meal site. 3. "Hill N Park" agrees to document the results of this outreach effort. "Hill N Park"understands that the grant of $1000 may be used to pay for the cost of the outreach program and to advance the overall "Hill N Park" senior citizens program. 4. "Hill N Park"understands this is a one time only incentive award and the terms of this agreement are valid through June 30, 2001. The "AAA"hereby agrees to award grant monies to "Fort Lupton" in the sum of$1,000. 2. "Fort Lupton" agrees to mail birthday and holiday greetings to approximately 800 local seniors. At least one mailing will include a senior information sheet for referral purposes. An open house will be planned for the opening of the new Fort Lupton Senior Center. 3. "Fort Lupton" agrees to document contacts made and senior attendees at the open house activity. IN WITNESS WHEREOF, the parties hereto have executed this Agreement on the day first written above.
