RE: APPROVAL OF SPECIAL EVENTS PERMIT ISSUED TO MAKE-A-WISH
FOUNDATION OF COLORADO, AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, Make-A-Wish Foundation of Colorado presented to the Board of County
Commissioners of Weld County, Colorado, an application for a Special Events Permit for the
sale of fermented malt beverages, containing not more than 3.2% of alcohol by weight, for
consumption by the drink on the premises only, and
WHEREAS, said applicant has paid the sum of TWENTY-FIVE DOLLARS ($25.00) to
Weld County, Colorado, pursuant to the laws providing therefore in payment for a County Retail
Permit for the sale of fermented malt beverages, containing not more than 3.2% of alcohol by
weight, for consumption by the drink on the premises only, for an event scheduled for ONE
DAY ONLY on May 20, 2001, from 8:00 a.m. until 9:00 p.m., outside the corporate limits of any
town or city in the County of Weld at the location described as follows:
Dandy Acres Painted Horse Farm, 1763 Weld County Road 20.5, Longmont, Colorado
Being more fully described as Lot B of Recorded
Exemption #2408; being part of the NE1/4 of
Section 17, Township 2 North, Range 68 West of
the 6th P.M., Weld County, Colorado
WHEREAS, said applicant has also paid the sum of TWENTY-FIVE DOLLARS ($25.00)
to the Colorado Department of Revenue and has exhibited a State Special Events Permit for
the sale of fermented malt beverages, containing not more than 3.2% of alcohol by weight, for
consumption by the drink on the premises only.
NOW, THEREFORE, BE IT RESOLVED that the Board of County Commissioners of
Weld County, Colorado, having examined said application and the other qualifications of the
applicant, does hereby grant Special Events Permit Number 2001-01 to said applicant to sell
fermented malt beverages, containing not more than 3.2% of alcohol by weight, for
consumption by the drink on the premises only, and the Board does hereby authorize and direct
the issuance of said permit by the Chair of the Board of County Commissioners, attested to by
the Clerk to the Board of Weld County, Colorado, which permit shall be effective from 8:00 a.m.
until 9:00 p.m. on May 20, 2001, providing that said place where the permittee is authorized to
sell fermented malt beverages, containing not more than 3.2% of alcohol by weight, for
consumption by the drink on the premises only, shall be conducted in strict conformity to all of
the laws of the State of Colorado and the rules and regulations relating thereto, heretofore
passed by the Board of County Commissioners of Weld County, Colorado, and any violations
thereof shall be cause for revocation of the permit.

SPECIAL EVENT PERMIT QUESTIONNAIRE AND AFFIDAVIT
1. What type of event is planned (e.g., annual, quarterly benefit, etc.)?
Frank's Ride for Children is an annual event and will be celebrating its 15th year on
Sunday May 20, 2001.
2. Explain in detail the nature of your organization, its function, and who or what benefits from
its operations (attach separate sheet if necessary).
The mission of Frank's Ride for Children is to help Colorado children with life-threatening illnesses
that want to have a wish granted. The children we sponsor participate in our run - riding in our
sidecars! They and their families enjoy seeing all the vehicles and motorcycles supporting their
wishes. All proceeds raised by Frank's Ride 2001 will be donated directly to Make-A-Wish
Foundation® of Colorado. (For more details, please reference the attached Make-A-Wish
Foundation® of Colorado information letter).
3. Who or what organization will be the recipient of funds derived from this event?
As stated in question 2,all proceeds raised by Frank's Ride will be donated directly to Make-A-
Wish Foundation® of Colorado.
4. How many attendees are expected at this event?
Approximately 1,000 - 1,500 people (740 attended this year's event).
5. Describe the premises at which the event will take place.
The event will be held at Dandy Acres Painted Horse Farm located just outside of Longmont,
Colorado at 1763 W.C.R. 20 I. This is a private 28-acre farm. The event will be on 10-15 acres of
the rear portion of the property.
6. What type of security will provided at this event?
The volunteer staff of Frank's Ride will provide security for this event. The staff members are
identified by wearing "FRANK'S RIDE STAFF" shirts.
7. How many security personnel will be on hand?
We had ten (10) designated "Security" staff members and thirty (30)Colorado National Guard
personnel on hand for this year's event. For next year's event, we plan to have thirty (30)
"FRANK'S RIDE SECURITY" designated staff and approximately one hundred (100) Colorado
National Guard personnel on hand.
8. How will security personnel be identified?
Security personnel will be identified by wearing "FRANK'S RIDE STAFF" shirts and SECURITY
armbands. The Colorado National Guard personnel are in uniform.
9. If the event is being held outdoors, how will the exterior boundaries of the premises be
marked (e.g., roped, fenced, etc.)?
The event will have exterior boundaries clearly marked with fencing and posted warning 'signs.
10. What type of entertainment will be provided for this event?
A local rock-n-roll band "Night Shift" will provide the entertainment.

11. What method will be used in checking identification for proper age of attendees (e.g., at
the door, at the bar, etc.) and how will underage patrons be identified so as not to be
served alcoholic beverages (e.g., stamp or mark on the hand, etc.)
The volunteer staff of Frank's Ride 2001 will be checking ID's at the door at the door and also at
the entrance of the event property. Legal age participants will receive wristbands provided by
Budweiser stating that they are allowed to consume alcoholic beverages. Underage participants
will receive wristbands provided by Budweiser that are clearly marked "NO ALCOHOL ALLOWED".
Signs will also be posted on the event property:
"No alcoholic beverages will be allowed off the premises" and "No one under 21 is permitted to
consume alcohol".
12. How will the conduct and level of intoxication of attendees be monitored and by whom?
The entire volunteer staff of Frank's Ride 2001 will monitor the conduct and level of intoxication
of event participants. There will also be Nextel radios on event premises for communication
between "FRANK'S RIDE SECURITY" and the rest of the staff members.
13. Have volunteers or members of your organization been trained in the sale/service of
alcoholic beverages: If yes, what training have they received and by whom?
Carol Rudzik, Nikki Rodgers and Jeff Rodgers have all completed the Budweiser training specified
"T.I.P.S." course. A refresher course was also given to this year's volunteer staff members.
Budweiser has agreed to provide a refresher course to the Frank's Ride 2001 staff as well.
14. What types of alternate beverages and food/snacks will be available?

The Make-A-Wish Foundation@ of Colorado was established in 1983 after the death of seven-
year-old Jennifer Mazak from liver disease. Although Jennifer did not have a formal wish, she
wanted to see the K MN Chicken. When the KIMN Chicken was told of Jennifer's wish, he
came to visit at her home. While Jennifer's appearance prompted gibes and stares from the other
neighborhood children, she was the envy of them all when the KIMN Chicken hugged and held
her and lavished attention on her.
The Colorado chapter is located in Denver and serves the entire state. We will have turned
dreams into reality for over 1,200 children in 1999, from Grand Junction to Burlington, from
Fort Collins to Trinidad and everywhere in between. Accomplishing our goals requires a
tremendous amount of volunteer effort and financial support. We are involved in year-round
fundraising and publicity efforts to ensure that as many eligible Colorado children as possible
know about Make-A-Wish®. All funds raised in Colorado are used in Colorado. Of every
dollar donated,approximately $.83 goes directly to wish granting.

The Make-A-Wish Foundation® of Colorado is not a United Way Agency. We do not receive
government funding and we do not solicit through telemarketing. We rely on the generosity of
individuals, corporations and foundations to ensure that wishes will continue to be granted as
long as there are children who qualify.
In 1999,the Make-A-Wish Foundation® of Colorado granted over 130 wishes to children
from all over the state. Wishes can range in cost from as little as $300 to as much as$10,000 or
more. However,the average combined cash and in-kind cost of a wish is approximately$6,000
and can range from a simple shopping spree,to a new bedroom, to meeting a popular athlete or a
Hollywood star,to a computer or a trip to Disney World or Hawaii. Because the granting of
wishes includes the child's entire family, approximately 450-600 people are touched each year.
Over 54% of wishes are for trips to Disney World regardless of the age of the child.
The impact of the lives of these Wish children has been tremendous. We have been told
numerous times by the families of Wish children that the realization that special wishes can
come true has made a difference in a child's attitudes,their families lives and even the child's
long-term health.
Childhood should be carefree and fun. Full of adventure. A time to dream. But for some
children, life is complicated, uncomfortable, painful and frightening. Each year,the lives of
thousands of children across the nation and their families are shattered by cancer, heart disease,
muscular dystrophy and other life-threatening illnesses. Their days are filled with doctors,
hospitals and shots. These children are asked to face medical treatments and prognoses that are
far removed from the wonder of normal childhood.
A fulfilled wish can replenish a child's faith in the basic wholeness and goodness of the world.
It lifts them up and carries them away to a place where dreams still do come true. It brings the
magic back, and with it...hope—as valuable a treatment as any medicine in the world.
Perhaps young Justin expressed it best when he wrote us:
"Whenever I felt like quitting, I just remembered my wish, swimming with the dolphins, and that
made things a lot more bearable for me and my family. "

Sponsored by Longmont Moose Lodge #1548 & Longmont Women of the Moose
Chapter #1211, Frank's Ride for Children benefits Make-A-Wish Foundation® of
Colorado and helps to grant wishes to Colorado children fighting life-threatening
illnesses. It is a 100+ mile poker run and winds through some of Colorado's most
beautiful scenery. The kids we sponsor participate in our run - riding in our
sidecars! They and their families have a lot of fun and really enjoy seeing all the
motorcycles and vehicles who show up to support them. Cost is $15.00 per person
(Adults 18 di over) and ($5.00 for children ages 12 to 18). This donation entitles
participants to one (1) poker hand (Adults 18 di over), door prize ticket, as well as,
Live Entertainment and a BBQ pork/beef roast at the Final Stop. All proceeds
raised by this event go directly to the Make-A-Wish Foundation® of Colorado.

3. As of 12-8-00, Make-A-Wish stated that their Insurance Company would in fact be naming
Weld County as an Additional Named Insured, per your request. We hope to have this

sending you the original documents naming Weld County as Additional Insured as soon as they
