QUESTIONNAIRE
Items 1,2,3,and 4- This existing developed residential lot has been in existence for over 100 years
and probably does not meet the intent of the Weld County Comprehensive
plan for agriculture zone. It is however very much involved with the
surrounding agricultural area.
The zone district for this site is agriculture and the setting is in a group of ten or
so older homes that are adjacent to Highland Lake. This little rural community
has been in existence for many years and with its many mature cottonwoods,
and its access to Highland Lake makes it a popular place for visitors. The
property is across the street from a small existing bait shop and snack bar
which is somewhat the center of the community. The historical"Little Mead
school house" fits nicely into this nucleus of the community and with the
historical Highland Lake church close by it does contribute to the historical
nature of the area.

NOTICE OF COMPLETION. The improvements on the property securing your loan are newly constructed;
therefore, a final inspection by our Appraiser is required prior to closing. Additionally, a Notice of

Completion and/or Certificate of Occupancy/Final Inspection Notice, or equivalent will be required at
closing.
RIGHT OF CANCELLATION (RESCISSION). You will have the right to cancel (rescind) this loan during
the 3 business days following the signing of your Loan Documents. At closing you will be given a
"Notice of Right to Cancel" which explains the terms of the cancellation period under Federal Regulation
Z, Truth in Lending. If you choose to cancel the loan, you must notify us in accordance with the
instructions contained in the Notice. FUNDS WILL NOT BE DISBURSED UNTIL THE END OF THE
3—DAY CANCELLATION PERIOD.

I, Sheryl Senesac, a certified elementary/early childhood teacher, and mother of five
children, have chosen to continue my teaching profession by teaching my own
children and children of close friends in a home environment. I am applying today
for a Special Use Permit so that I can continue to teach at my home.
Since I do not work outside the home, I have found it enjoyable to be able to teach
my children and others art classes and preschool skills in my home. I have been
doing this on a very small scale for the past 13 years. We moved to our current
residence in 1995 and have been enjoying our historical home tremendously. On our
property is Mead's original schoolhouse, which was moved from its original location
by the Highland Lake Church to our property in 1915. Excitedly,we decided to
renovate the old school building, and with the help of friends and family we slowly
made the building again useable. It is now a building used for a very small number
of children on a small scale. I use the building three days a week, from 9:00-12:00,
in the months of September through May. I feel it has been a safe, Christian haven
away from home. Our children also enjoy the use of this building.
It was never our intention to circumvent the planning process, not realizing that a
special review was necessary. We appreciate your time in considering this late
application.
