RE: APPROVE AMENDMENT TO AGREEMENT FOR JAIL MEDICAL UNIT SERVICE AND
AUTHORIZE CHAIR TO SIGN -CORRECTIONAL HEALTHCARE MANAGEMENT,
INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Agreement for
Jail Medical Unit Service between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Sheriff's Office, and
Correctional Healthcare Management, Inc., commencing upon full execution of said
amendment, and ending December 31, 2001, with further terms and conditions being as stated
in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the Agreement for Jail Medical Unit Service
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Sheriff's Office, and Correctional Healthcare
Management, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 15th day of November, A.D., 2000.

RE: Amendment for jail medical services for year 2001
Dear Mr. Fliethman:
Pursuant to our Agreement dated January 1, 1999, enclosed pleased two (2) copies of our
Amendment for jail medical services for the term January 1, 2001 to December 31, 2001.
Correctional Healthcare Management, Inc. submitted a request for an increase in a letter dated
May 16, 2000.
Please have both copies of the Amendment signed and approved and forward one copy back to
our office for our records. If you have any questions, please feel free to call me at 303-706-
9080. Our corporate office has recently moved. Our new address is 7967 Witney Place,
Littleton, Colorado 80124.
We look forward to providing another year of quality inmate health care services for the Weld
County Jails.

AMENDMENT TO
AGREEMENT FOR JAIL MEDICAL UNIT SERVICE
THIS AMENDMENT, dated for reference purposes only, the 2nd day of November, 2000 is made and entered into by and
between the Board of County Commissioners of the County of Weld, 915 Tenth Street, Greeley, Colorado 80631 (hereinafter
"County"), and Correctional Healthcare Management, Inc., (hereinafter "Provider"). It is hereby attached to and shall
become a part of the Agreement by and between County and Provider, which was executed January 1, 1999.
WITNESSETH
WHEREAS,County and Provider entered into a Jail Medical Unit Service Agreement("Agreement")January 1, 1999.
WHEREAS, Provider desires the Agreement be renegotiated with the following changes, subject to the approval of County.
NOW THEREFORE, for good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged,
County and Provider agree to the following changes to the Agreement:
1. The monthly compensation payable to Provider in the amount of$50,208.34 is hereby adjusted to $52,216.67
for the renewed period January 1, 2001 through December 31, 2001. Pursuant to the original Agreement,
Section XVIII, titled Renegotiation or Modification on page 9, in which a letter was submitted to Weld
County on June 14, 1999 requesting an increase for the year 2001.
2. The Agreement, as amended hereby, shall remain in full force and effect through December 31, 2001.
County and Provider have each caused this agreement to be executed by their duly authorized officers as of the date first
written above.
