RE: APPROVE CHANGE ORDER LETTER#1 FOR EPSDT PROGRAM CONTRACT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 for the EPSDT
Program Contract between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment, with
terms and conditions being as stated in said change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#1 for the EPSDT Program Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 5th day of July, A.D., 2000.

SUBJECT: Change Order Letter No. 1 to the EPSDT Contract
DATE: June 29, 2000
Enclosed for Board review and approval is Change Order Letter#1 to the contract between the
Colorado and Weld County Departments of Public Health and Environment for the EPSDT
Program.
This change order letter is intended to be effective as of June 15, 2000, and it authorizes Weld
County to purchase and be reimbursed for three computers, software and a laser jet printer. The
equipment is necessary for WCDPHE to implement the new IRIS data tracking system which
will be required for the EPSDT program.
For these purchases, WCDPHE will be reimbursed an amount not to exceed $9,035. I
recommend your approval of this change order letter.
