RE: GRANT PERMIT FOR TEMPORARY ASSEMBLY TO ROBERT AND CAROL RUDZIK

WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, Robert and Carol Rudzik, 18 8th Avenue, Longmont, Colorado, have
presented to the Board of County Commissioners an application for a Permit for Temporary
Assembly on land within the unincorporated portion of the County of Weld, State of Colorado, to
be held from 8:00 a.m. until 9:00 p.m. on May 20, 2001, on property described as follows:
Dandy Acres Painted Horse Farm, 1763 Weld County Road 20.5,
Longmont, Colorado 80504; being more particularly described as
located on Lot B of Recorded Exemption #2408; being part of the
NE1/4 of Section 17, Township 2 North, Range 68 West of the 6th
P.M., Weld County, Colorado
WHEREAS, said applicant has paid to the Weld County the sum of ONE-HUNDRED
DOLLARS ($100.00) for said Weld County Permit for Temporary Assembly, and
WHEREAS, having examined said application, the Board deems it appropriate to grant
a Weld County Permit for Temporary Assembly to Robert and Carol Rudzik, and
WHEREAS, said permit shall be issued subject to compliance with the terms and
conditions as set forth in the application materials and upon the express condition and
agreement that the temporary assemblage will be conducted in strict conformity to all laws of
the State of Colorado, and
WHEREAS, any violation to said terms and conditions shall be cause for revocation of
said permit and forfeiture of bond.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that said Weld County Permit for Temporary Assembly on May 20,
2001, be, and hereby is, granted to Robert and Carol Rudzik to conduct a temporary
assemblage at the hereinabove described location.

WELD COUNTY PERMIT FOR TEMPORARY ASSEMBLY
By action of the Board of County Commissioners of the County of Weld, State of Colorado,
taken this 18th day of December, 2000, IT IS HEREBY GRANTED a permit for a temporary
assemblage on land within the unincorporated portion of the County of Weld, State of Colorado,
to be held from 8:00 a.m. until 9:00 p.m.on the 20th day of December, 2001, on property described
as:
Dandy Acres Painted Horse Farm, 1763 Weld County Road 20.5, Longmont,
Colorado 80504; being more particularly described as located on Lot B of Recorded

This permit authorizes Robert and Carol Rudzik to hold such an assemblage so long as it
is in accordance with Weld County Ordinance No. 85 and all other applicable State and County
statutes and regulations. This permit is subject to compliance with the terms and conditions as set
forth in the application materials and to additional conditions set forth by the Board and attached
hereto as Exhibit "A."

This is a private 28-acre farm located just outside of Longmont, Colorado and the event
will be held on the rear portion of the property.

2. Nature of purposes of the assembly:
The mission of Frank's Ride is to help Colorado children who are fighting life-threatening
illness and who want to have a wish granted. The children we sponsor participate in our
run - riding in our sidecars! They and their families have a great time and really enjoy
seeing all the motorcycles and vehicles supporting their wishes.
3. Dates and hours during which the assembly is to be held:
Frank's Ride will be celebrating its 15th year on Sunday May 20, 2001. This event will start
at 8am and last until approximately 9pm on 5-20-2001.
4. Maximum number of persons to be allowed at assembly at any one time:
The total number of participants at the 14th Annual Frank's Ride held 5-20-00 was 740;
therefore, we expect approximately between 1,000 - 1,500 to attend the 2001 event.
5. The maximum number of tickets to be sold, if any:
Not applicable. Participants pay their donation fee on the day of the event at Sign-up /
Registration. These proceeds are then donated directly to the Make-A-Wish Foundation®
of Colorado.

6. Identify plans to limit the number of persons to the maximum identified above:
See above - not applicable.
7. Identify plans for supplying potable (drinking) water, including the source, number and
location of facilities:
The Colorado National Guard (located in Longmont di Thornton, Colorado) donated 1,000
bottles of drinking water and has agreed to donate the same for Frank's Ride 2001. They
stated that they would increase the amount of water to donate per our request. McLane
Western, Inc. located in Longmont, Colorado donated 240 bottles of drinking water and
also agreed to donate to Frank's Ride 2001. The drinking water will be placed in horse
troughs (in ice) in various shaded areas of the event area.
8. Identify the plans for providing separate toilet facilities for males and females,
including the source, number and location, type and means of disposing of wastes
deposited:
Waste Management of Northern Colorado provided three (3) regular outhouses, as well as,
one (1) handicap access outhouse at this year's event. Frank's Ride also owns two (2)
additional outhouses. The total number of outhouses provided were five (5) regular
outhouses and one (1) handicap access outhouse. Waste Management provided delivery,
set-up and removal of the outhouses/waste from the property. They have agreed to
provide the same service for Frank's Ride 2001. As we are expecting more people, we will
provide additional outhouses for next year's event - seven (7) regular and two (2) handicap
access outhouses. The outhouses will be clearly marked MALE and FEMALE.
9. Identify the plans for holding, collecting and disposing of solid waste materials:
As mentioned above, Waste Management of Northern Colorado will be providing this
service for Frank's Ride 2001.
10. Identify the plans to provide for medical facilities, including the location and
construction of any structures, the names, addresses and hours of availability of
emergency medical technicians and nurses and provisions for emergency ambulance
service:
Longmont Emergency Unit provided emergency ambulance/medical service at this year's
event. The ambulance and medical technicians were located on the property for the entire
duration of the event and have agreed to provide Frank's Ride 2001 with the same service.
11. Identify the plans, if any, to illuminate the location of the assembly, including the
source and amount of power, and the location of lamps:
Sam Jones, Captain of the Electric Department of PRATT Company provided the power for
this year's event and will do so again in 2001. The power pole is located at the right of the
Stage Area (see diagram). A 200amp generator provided the power necessary to run lights
to illuminate the Stage, Food and Bathroom areas.

12. Identify the plans for parking vehicles, including the size and location of lots,
points of highway access and interior roads, including routes between highway access
and parking lots:
See the diagram for Frank's Ride 2001. As this is a private horse farm, there are no
parking lots or highway access; however, the parking areas will be clearly marked and
Frank's Ride Staff members will direct participants into the designated parking areas (this
was done at this year's event and will be done again for Frank's Ride 2001).
13. Identify the plans for communications with hospital, police and fire services,
including the source, amount and location of communication equipment:
See questions 10, 15 and 16.
• Longmont Emergency Unit personnel will be providing medical services and will be
present on the event premises.
• Mountain View Fire District personnel will be providing fire protection for this event.
• Longmont Police Department will have personnel available.
14. Identify the plans for camping facilities, if any: Not applicable.
15. Identify the plans for fire protection.
Mountain View Fire District located at County Road 1 and W.C.R. 20 ₹ will be providing
fire protection for Frank's Ride 2001.
16. Identify the plans for security, including the number of guards, their deployment,
their names, addresses, credentials and hours of availability and description of peer
group control, if any:
The volunteer staff of Frank's Ride will provide security for the 2001 event, as it was done
at this year's event. The staff members are identified by wearing "FRANK'S RIDE STAFF"
shirts. Additionally, there will be thirty (30) staff members designated as "Security" and
will be identified by wearing "STAFF" shirts and "SECURITY" armbands. The Colorado
National Guard also assisted with Security at this year's event and have agreed to do so
again at Frank's Ride 2001.
17. Identify the plans for sound control and sound amplification, if any, including
number, location and power of amplifiers and speakers:
Night Shift, a local Denver band, provided Live Entertainment for this year's event. They
had three (3) 20amp circuits located at the Stage Area (see diagram)and Taylor Ball, LLC
provided two (2) 200amp generators for the Band's power needs. This will be done again
for Frank's Ride 2001.

18. If applicable, identify plans for meeting County health standards for food
concessions and concessionaires who will be allowed to operate on the grounds,
including the names and addresses of all concessionaires and their license permit
numbers:
Not applicable - we do not allow food concessions and/or food concessionaires. The
volunteer staff of Frank's Ride prepares and serves the BBQ pork/beef, which is roasted in
four (4) large cast iron cookers.
19. Identify the plans, if any, for an electrical system:
See questions 11 and 17. PRATT, Inc. and Taylor Ball, LLC will be providing the set-up and
use of their job-site-sized generators to provide the electricity for our event.
20. Identify the plans to ensure that trees, underbrush, large rocks and other natural
features shall be left intact and undisturbed, and natural vegetative cover retained,
protected and maintained so as to facilitate drainage, prevent erosion and preserve
the scenic attributes; and the plans to abate dust on the site:
Bill Bowen, Owner of Centennial Backhoe located in Longmont, Colorado has provided
personnel and equipment to Frank's Ride for many years. He and his personnel are very
committed to the preservation of all scenic attributes of the event premises and work very
hard to maintain the property's natural state. As mentioned in question 1, the event is
being held on 10-15 acres in the rear portion of the property. Several staff members of
this year's event (Jeff Rodgers, Doug Keim and Tracy Wright)recently planted 75 pounds
of pastureland grass seed at the property to regenerate any grass damaged by the event
participants. This will also assist in abating any dust on the property, as well as, prevent
erosion while maintaining the property's scenic attributes. The same will be done for and
after next year's event.
21. Attach a statement by a bonding company licensed to do business in the State of
Colorado indicating its intent to furnish a bond to ensure performance by the applicant
of each of the terms and conditions of the temporary assemblage permit.
Make-A-Wish Foundation® of Colorado has agreed to cover Frank's Ride 2001 under their
bonding company. (More information will be provided under separate cover).
22. Attach a statement by an insurance company licensed to do business in the State
of Colorado stating its intent to provide liability insurance to protect against injury to
persons or property occurring as a result of such an assembly.
Make-A-Wish Foundation® of Colorado has agreed to cover Frank's Ride 2001 under their
event insurance policy. (More information will be provided under separate cover).

On December 18, 2000, the Board approved a Temporary Assembly Permit for Robert and
Carol Rudzik, for events to be held on May 20, 2001. At the request of the County Attorney's
Office, Mr. Rudzik called the Clerk to the Board's Office and stated they will be conducting the
poker run events at the Longmont Moose Lodge, which has been issued Bingo License #2001-
0092.
