RE: APPROVE FOUR PURCHASE OF SERVICES AGREEMENTS FOR VARIOUS RURAL
SENIOR CENTERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with four Purchase of Services Agreements
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the following rural Senior Centers:
1) Nunn Senior Citizens
2) Pierce Senior Citizens
3) Hill-N-Park Seniors, Inc.
4) Lochbuie After Glows
WHEREAS, after review, the Board deems it advisable to approve said agreements,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the four Purchase of Services Agreements between the County of
Weld. State of Colorado, by and through the Board of County Commissioners of Weld County.
on behalf of the Department of Human Services, Area Agency on Aging, and the above listed
rural Senior Centers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of July, A.D., 2000.

SUBJECT: Purchase of Service Agreements for Senior
Center Operations
Enclosed for Board approval are Purchase of Service Agreements with four rural
Senior Centers - Nunn, Pierce, Hill N Park, and Lochbuie. Each of these centers faces
financial hardship in terms of keeping their centers open. The centers will utilize the
grants to help pay for heat, lights, telephone, supplies, water and other operational
expenses. The following grants are to be awarded:
• Nunn Senior Center - $2,500
• Pierce Senior Center - $3,000
• Hill N Park Senior Center- $3,000
• Lochbuie Senior Center - $2,000

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 24th day of July, 2000 by and between the County of Weld.
State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of tt le Weld
County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Nunn
Senior Citizens of Nunn hereinafter referred to as "Nunn Seniors."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, "AAA" considers small rural senior centers a vital link in the county's system of services to older
adults, and
WHEREAS, "Nunn Seniors" is a senior center providing recreational, nutritional, and educational opportunities
for senior citizens;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies (under the Older Americans Act) in the sum of S2 5011 to
"Nunn Seniors."
2. "Nunn Seniors" agree to expend the grant monies to pay for operating expenses for the center to include:
electricity, gas, telephone, insurance, office supplies, postage, and other minor operating expenses
3. "Nunn Seniors" agree to maintain documentation of the above expenditures and to seek prior approval from
the Area Agency on Aging if it is needed to expend a portion of this grant on expenditures not listed in
number (2) above.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement on the day first written above.

from the Area Agency on Aging if it is needed to expend a portion of this grant on expenditures not listed
in number (2) above.

the Area Agency on Aging if it is needed to expend a portion of this grant on expenditures not listed in
number (2) above.

to "Lochbuie After Glows."
2. "Lochhuie After Glows" agree to expend the grant monies to pay for operating expenses for the .:enter to
include: electricity, gas, telephone, insurance, office supplies,postage, and other minor operating expenses.
3. "Lochhuie After Glows" agree to maintain documentation of the above expenditures and to seek prior
approval from the Area Agency on Aging if it is needed to expend a portion of this grant on expenditures
not listed in number (2) above.
