RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR CHLAMYDIA TESTING
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Change Order Letter 41 for
Chlamydia Testing between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing August 1, 2000, and ending December 31, 2000, with further terms and conditions
being as stated in said task order change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
change order letter, a copy of which is attached hereto and incorporated herein by reference
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Change Order Letter#1 for Chlamydia Testing
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of July, A.D., 2000.

SUBJECT: Task Order Change Order Letter#1 for Chlamydia Testing
DATE: July 24, 2000
Enclosed for Board review and approval is Change Order Letter#I for the Chlamydia'List rig
Task Order between the Weld County Department of Public Health and-Environment
(WCDPHE) and the Colorado Department of Public Health and Environment.
This change order letter provides an additional $624 in funding fiir Chlamydia tests done in the
sexually transmitted disease and family planning elirics. With this additional funding, ;he toad
amount WCDPHE will be reimbursed for Chlamydia tests for the time period January I. 20(10
through December 31, 2000 is $710.5.
I recommend your approval of this change order letter.

Pursuant to paragraph 5 of Section F of Master Contract with contract routing number 00 FAA 00008,and l ask
Order with contract routing number 00 FHA 00148 and contract encumbrance number PO FHA EPI000014$,
hereinafter referred to as the"Original Task Order"(a copy of which is attached hereto and by this reference
incorporated herein and made a part hereof)between the State of Colorado,Department of Public Health and
Environment and WELD COUNTY HEALTH DEPARTMENT for the renewal term from January 1.20O,
through December 31.2000,the parties agree that the maximum amount payable by the State for the eligible
services referenced in paragraph I of section C of the Original Task Order is increased by SIX HUNDRED
TWENTY FOUR DOLLARS,($624.00)for a new total financial obligation of the State of SEVEN THOUSAND
ONE HUNDRED FIVE DOLLARS, ($1,010)in accordance with the budget outlined below. The first sentence
in paragraph 1 of section C of the Original Task Order is hereby modified accordingly. All other terms and
conditions of the Original Task Order are hereby reaffirmed. This amendment to the Original Task Order is
intended to be effective as of August 1.2000. However, in no event shall this amendment be deemed valid until it
stall have been approycd by the State Controller otsuch assistant as he may designate.
Chlamydia Testing:
A. STD Clinic: 120 @$7.80 each $ 936.00
B. Family Planning Clinic: 730@$7.80each $5,694.00
Chlamydia Treatment Drugs:
Azithromycin for STD Clinic and
Family Planning Clinic: 50 doses @$9.50 each $ 475,00
