RE: APPROVE MEMORANDUM OF AGREEMENT FOR FAMILY PLANNING PROGRAM
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Agreement for the
Family Planning Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing February 23, 2000, and ending June 30, 2000, with further terms
and conditions being as stated in said memorandum of agreement, and
WHEREAS, after review, the Board deems it advisable to approve said Memorandum of
Agreement, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Memorandum of Agreement for the Family Planning Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said memorandum of agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of March, A.D., 2000, nunc pro tunc February 23, 2000.

SUBJECT: Memorandum of Agreement with the Colorado Department of
Public Health and Environment
DATE: March 16, 2000
Enclosed for Board review and approval is a memorandum of agreement between Weld County
Department of Public Health and Environment and the Colorado Department of Public Health
and Environment Women's Health Section for the Family Planning Program.
With this agreement, Weld County is being awarded $3,350 to provide sterilization procedures
and $975 to provide cryotherapy services for a total of$4,325. These funds are provided as part
of the Family Planning Program's Reduction in Unintended Pregnancy Initiative.
This agreement will be in effect for the time period February 23, 2000 through June 30, 2000,
and
Health Department staff recommends your approval.

Between the Colorado Department of Public Health and Environment,Women's Health Section
and
Weld County Health Department
For the Period February 23, 2000 through June 30, 2000
This document details the services to be provided as part of the Family Planning Program -
Reduction in Unintended Pregnancy Initiative. Weld County Health Department has been
awarded $3,350 for sterilizations and $975 to provide cryotherapy services for a total of$4,325.
Funding for the project begins February 23, 2000 and ends June 30, 2000. Billing will be
submitted by the June 30, 2000 completion date unless other arrangements are made.
