RE: APPROVE CONTRACT RENEWAL LETTER#1 FOR HEALTH CARE PROGRAM FOR
CHILDREN WITH SPECIAL NEEDS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter#1 for the
Health Care Program for the Children with Special Needs Program between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Department of Public Health and Environment, and the Colorado
Department of Public Health and Environment, commencing July 1, 2000, and ending
September 30, 2000, with further terms and conditions being as stated in said contract renewal
letter, and
WHEREAS, after review, the Board deems it advisable to approve said contract renewal
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract Renewal Letter#1 for the Health Care Program for
the Children with Special Needs Program between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 12th day of June, A.D., 2000.

Pursuant to paragraph C5 of the contract with contract routing number 00-00191 and contract encumbrance number PO
FAA HCP0000191,hereinafter referred to as the"Original Contract" (a copy of which is attached hereto and by this
reference incorporated herein and made a part hereof) between the State of Colorado, Department of Public Health and
Environment and Board of County Commissioners of Weld County, for the renewal term from July 1, 2000, through
September 30,2000, the parties agree that the maximum amount payable by the State for the eligible services referenced in
paragraphs BI through B19 of the Original Contract is increased by THIRTY-NINE THOUSAND THREE HUNDRED
FIFTY-SEVEN DOLLARS, ($39,357,00)for a new total financial obligation of the State of TWO HUNDRED TWO
THOUSAND FIVE HUNDRED THIRTY-FIVE DOLLARS, ($202,535.00). The revised budget, which is attached
hereto as"Attachment I", and the revised annual statement, which is attached hereto as"Attachment 2", are incorporated
herein by this reference and made a part hereof. The first sentence in paragraph C2 of the Original Contract is hereb
modified accordingly. All other terms and conditions of the Original Contract are hereby reaffirmed. This amendm<n- to
the Original Contract is intended to be effective as of July 1,2000. However, in no event shall this amendment be do cined
valid until it shall have been approved by the Stale Controller or such assistant as he may designate.
Please sign, date, and return all 4 originals of this Contract Renewal Letter by June 7,2000, to the attention of: Peggy:

Renewal Letter#1
DATE: June 9, 2000
Enclosed for Board review and approval is task order renewal letter#1 for the Health Care
Program for Children with Special Needs Program. If approved, this renewal letter will becoruv
a part of the master contract between the Colorado Department of Public Health and
Environment and the Weld County Department of Public Health and Environment (WCDPHE
This renewal letter is being issued by the Colorado Department of Public Health and
Environment to extend the period for the current task order by three months. Instead of the tat.
order ending on June 30, 2000, it will now end on September 30, 2000. The renewal letter also
adds another $39,357 to the funding of the original task order. This funding will allow
WCDPHE to continue the provision of case finding, community outreach, care coordinat ion,
clinic management, program management, and interagency collaboration to children and their
families who are determined to be eligible for services.
