HomeMy WebLinkAbout20001419.tiff RESOLUTION RE: APPROVE CONTRACT RENEWAL LETTER#1 FOR HEALTH CARE PROGRAM FOR CHILDREN WITH SPECIAL NEEDS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Contract Renewal Letter#1 for the Health Care Program for the Children with Special Needs Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing July 1, 2000, and ending September 30, 2000, with further terms and conditions being as stated in said contract renewal letter, and WHEREAS, after review, the Board deems it advisable to approve said contract renewal letter, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Contract Renewal Letter#1 for the Health Care Program for the Children with Special Needs Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said contract renewal letter. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 12th day of June, A.D., 2000. If approved, this renewal letter will becoruv a part of the master contract between the Colorado Department of Public Health and Environment and the Weld County Department of Public Health and Environment (WCDPHE This renewal letter is being issued by the Colorado Department of Public Health and Environment to extend the period for the current task order by three months. Instead of the tat. order ending on June 30, 2000, it will now end on September 30, 2000. The renewal letter also adds another $39,357 to the funding of the original task order.
