HomeMy WebLinkAbout20001536 RESOLUTION RE: APPROVE CLOSEOUT AUTHORIZATION FORM FOR FISCAL YEAR 1999-2000 AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Closeout Authorization Form for Fiscal Year 1999-2000 between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and the Colorado Department of Human Services, with terms and conditions being as stated in said form, and WHEREAS, after review, the Board deems it advisable to approve said form, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the Closeout Authorization Form for Fiscal Year 1999-2000 between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and the Colorado Department of Human Services be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said form. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 26th day of June, A.D., 2000. The Colorado Department of Human Services is using this form to meet § 26-2-714, to have county authorization to transfer funds for purposes of Core Services and Child Welfare expenditures among the allocation blocks. These allocation blocks include the Child Welfare Block, Child Care Development Block, and the Colorado Works or Temporary Assistance for Needy Families (TANF). If you have any questions, please telephone me at extension 6510. ji Il 0 1531 Closeout Authorization Form FY 1999-00 County Weld County Department of Social Services This form will be used as authorization in determining individual county close out options for FY 1999-00 ONLY. For each section please rank in order of how the county like the State to close the county for FY 1999-00. Section 1 Core Services (Will be closed out first) 3 transfer funds from Child Welfare Block 2 transfer fund from TANF if all criteria are met. 1st to Core Services for Title XX services then to Child Care Development Block. _1_ transfer funds from TANF (if all criteria are met 1st to Child Care Development Block then to Core Services for Title XX services 4 county only Section 2 Child Welfare (Will be closed out second) 2 _ transfer funds from TANF if all criteria are met. 1st to Child Welfare for Title XX services then to Child Care Development Block 1 transfer funds from TANF if all criteria are met 1st to Child Care Development Block then to Child welfare for Title XX services 3 county only Please fax this form to: Julie Perez, Child Welfare Division, Fax Number 303-866-5563 Approval: Barbara J.
