RE: APPROVE AMENDMENT TO VOLUNTARY GROUP SHORT TERM DISABILITY
INSURANCE POLICY NUMBER 22125 AND AUTHORIZE CHAIR TO SIGN - ROCKY
MOUNTAIN LIFE INSURANCE COMPANY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Co orado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Voluntary Group
Short Term Disability Insurance Policy Number 22125 between the County of Weld, State of
Co orado, by and through the Board of County Commissioners of Weld County, and Rocky
Mountain Life Insurance Company, with terms and conditions being as stated in said
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the Voluntary Group Short Term Disability
Insurance Policy Number 22125 between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, and Rocky Mountain Life
Insurance Company be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 30th day of October, A.D., 2000.

Per your request, we are amending the number of hours to 20, effe:tive October 1, 2000.
Enclosed are two (2) copies of an amendment to your Voluntary Group Short Term Disability
Insurance Policy issued by Rocky Mountain Life Insurance Company. Please have an officer of
the company sign and date both copies.
One copy should be returned to us on or before November 13, 2000, in the enclosed return
envelope. The other should be attached to the Master Policy originally issued to you.
Also enclosed are Certificate Amendments for your employees. Please distribute these
amendments to those employees who have this coverage and instruct them to attach it to tlu it
booklet.

ELIGIBLE REGULATIONS
Delete the following language:
"z All Employees working a minimum of thirty (30) hours per week"
and replace with the following language:
"z Other: All Employees working a minimum of twenty (20) hours per week"
IN ALL OTHER RESPECTS THIS POLICY REMAINS UNCHANGED.
