VISITATION AND AUTHORIZE CHAIR TO SIGN -WELD COUNTY DEPARTMENT OF
SOCIAL SERVICES
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Notification of Financial Assistance
Award for Lifeskills -Visitation between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, and the Department of Social Services,
commencing June 1, 2001, and ending May 31, 2002, with further terms and conditions being
as stated in said award, and
WHEREAS, after review, the Board deems it advisable to approve said award, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Notification of Financial
Assistance Award for Lifeskills -Visitation between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, and the Department of Social
Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said award.
The above and foregoing Resolution was, on motion duly made and seconded, adopted

Enclosed for Board approval are the PY 2001-2002 Notifications of Financial Assistance
Awards (NOFAA) for Families, Youth, and Children Commission (FYC) Core Services
Funds, which are for the period of June 1, 2001,through May 31, 2002.
The Families, Youth and Children Commission (FYC) reviewed proposal's under a
Request for Proposal process and are recommending approval of these bids.
Weld County Department of Social Services
A. Lifeskills-Visitation: Capacity is seven to ten families, or approximately 30
people per month per worker. Children ages birth through 18 are eligible for
transportation. Bicultural-bilingual services are available to approximately 50
percent of the clients. A primary focus of the service is South County. Providers
will be paid$11/hr. Additional reimbursement will include a benefit rate of 18
percent, or$1.98 per hour. Mileage will be paid per Weld County policy, but out
of a separate funding source.
B. Sex Abuse Treatment. Depending on the gravity of the presenting issues, there
will be variability as to how much contact is required on a weekly basis. Program
capacity and the total number of clients to be served will be flexible. Provider

The Families, Youth and Children Commission, an advisory commission to the Weld County Department of
Social Services, announces that applications will be accepted for approved vendors pursuant to the Board of
Weld County Commissioners' authority under the Statewide Family Preservation Program (C.R.S. 26-5.5-
101) and Emergency Assistance for Families with Children at Imminent Risk of Out-of-Home Placement
(C.R.S. 26-5.3-101). The Families, Youth and Children Commission wishes to approve services targeted to
run from June 1, 2001, through May 31, 2002, at specific rates for different types of service, the county will
authorize approved vendors and rates for services only. The Life Skills Program must provide services that
focus on teaching life skills which are designed to improve household management competency,parental
competency, family conflict management and effectively accessing community resources. This program

TERMS AND CONDITIONS
RFP-FYC-01005 through 01011
1. The Contractor agrees it is an independent contractor and that its officers and employees do not
become employees of Weld County, nor are they entitled to any employee benefits as Weld County
Employees if this RFP/Bid is accepted by the Board of County Commissioners.
2. Weld County, the Board of County Commissioners of Weld County, its officers and employees,
shall not be held liable for injuries or damages caused by any negligent acts or omissions of
Contractor or its employees, volunteers, or agents while performing duties as described pursuant to
this RFP/Bid. Contractor shall indemnify, defend, and hold harmless Weld County, the Board of
County Commissioners of Weld County, its employees, volunteers, and agents. The Contractor
shall provide adequate liability and workers' compensation insurance for all its employees,
volunteers, and agents engaged in the performance as prescribed under the RFP/Bid.
3. No portion of this Bid shall be deemed to constitute a waiver of any immunities the parties or their
officers or employees may possess, nor shall any portion of the RFP/Bid be deemed to have created
a duty of care with respect to any persons not a party to this RFP/Bid.
4. No portion of this RFP/Bid shall be deemed to create an obligation on the part of the County of
Weld, State of Colorado, to expend funds not otherwise appropriated in each succeeding year.
5. If any section, subsection, paragraph, sentence, clause, or phrase of this RFP/Bid is for any reason
held or decided to be unconstitutional, such decision shall not affect the validity of the remaining
portions. The parties hereto declare that they would have entered into this RFP/Bid and each and
every section, subsection, paragraph, sentence, clause, and phrase thereof irrespective of the fact
that any one or more sections, subsections, paragraphs, sentences, clauses, or phrases might be
declared to be unconstitutional or invalid.
6. No public official or employee of Weld County, Colorado, and no member of their governing
bodies shall have any pecuniary interest, direct or indirect, in the approved RFP/Bid or the
proceeds thereof.
7. The Contractor assures that they will comply with the Title VI of the Civil Rights Act of 1986 and
that no person shall, on the grounds of race, creed, color, sex, or national origin, be excluded from
participation in, be denied the benefits of, or be otherwise subjected to discrimination under an
approved RFP/Bid.
8. The Contractor assures that sufficient, auditable, and otherwise adequate records that will provide
accurate, current, separate, and complete disclosure of the status of the funds received under the
RFP/Bid are maintained for three (3) years or the completion and resolution of an audit. Such
records shall be sufficient to allow authorized local, federal, and state auditors and representatives
to audit and monitor the Contractor.
9. The Contractor assures that authorized local, federal, and state auditors and representatives shall,

during business hours, have access to inspect and copy records, and shall be allowed to monitor
and review through on-site visits, all contract activities, supported with funds under this RFP/Bid
to ensure compliance with the terms of this RFP/Bid. Contracting parties agree that monitoring
and evaluation of the performance of the RFP/Bid shall be conducted by appropriate funding
sources. The results of the monitoring and evaluation activities shall be provided to the appropriate
and interested parties.
10. A RFP/Bid which is approved by the Board of County Commissioners of Weld County, shall be
binding upon the parties hereto, their successors, heirs, legal representatives, and assigns and shall
constitute a contract without further action by the Board. The Contractor or Social Services may
not assign any of its rights or obligations hereunder without the prior written consent of both
parties.
11. The Contractor assures that it will fully comply with the Weld County Family Preservation
Program regulations promulgated, and all other applicable federal and state laws, rules and
regulations. The Contractor understands that the source of funds to be used under this RFP/Bid is:
Family Preservation/Family Support Funds of the Omnibus Budget Reconciliation A t of 1993.
12. The Contractor assures and certifies that it and its principals:
a. Are not presently debarred, suspended, proposed for debarment, declared ineligible, or
voluntarily excluded from covered transactions by a Federal department or agency;
b. Have not, within a three-year period preceding this RFP/Bid, been convicted of or had a
civil judgment rendered against them for commission of fraud or a criminal offense in
connection with obtaining, attempting to obtain, or performing a public(Federal, state, or
local) transaction or contract under a public transaction; violation of federal or state
antitrust statutes or commission of embezzlement, theft, forgery, bribery, falsification or
destruction of records, making false statements, or receiving stolen property;
c. Are not presently indicted for or otherwise criminally or civilly charged by a government
entity (federal, state, or local) with commission of any of the offenses enumerated in
paragraph 12(b) of this certification; and
d. Have not within a three-year period preceding this RFP/Bid, had one or more public
transactions (federal, state, or local) terminated for cause or default.
13. The Appearance of Conflict of Interest applies to the relationship of a contractor with Social
Services when the contractor also maintains a relationship with a third party and the two
relationships are in opposition. In order to create the appearance of a conflict of interest, it is
not necessary for the contractor to gain from knowledge of these opposing interests. It is only
necessary that the contractor know that the two relationships are in opposition.

During the term of this RFP Bid, if it is approved by the Board of County Commissioners, the
contractor shall not enter into any third party relationship that is a conflict of interest or gives the
appearance of creating a conflict of interest. Upon learning of an existing appearance of a conflict
of interest situation, the contractor shall submit to Social Services a full disclosure statement
setting forth the details that create the appearance of a conflict of interest. Failure to promptly
submit a disclosure statement required by this paragraph shall constitute grounds for Social
Services' termination, for cause, of their contract with the contractor.
14. Contractor shall protect the confidentiality of all applicant records and other materials that are
maintained in accordance with this RFP/Bid, if it is approved by the Board of County
Commissioners. Except for purposes directly connected with the administration of the Family
Preservation Program, no information about or obtained from any applicant/recipient in possession
of Contractor shall be disclosed in a form identifiable with the applicant/recipient or a minor's
parent or guardian. Contractor shall have written policies governing access to, duplication and
dissemination of, all such information. Contractor shall advise its employees, agents and
subcontractors, if any, that they are subject to these confidentiality requirements. Contractor shall
provide its employees, agents, and subcontractors, if any, with a copy or written explanation of
these confidentiality requirements before access to confidential data is permitted.
15. Proprietary information for the purposes of this contract is information relating to a party's
research, development, trade secrets, business affairs, internal operations and management
procedures and those of its customers, clients or affiliates, but does not include information (I)
lawfully obtained from third parties, (2) that which is in the public domain, or (3) that which is
developed independently.
Neither Party shall use or disclose directly or indirectly without prior written authorization any
proprietary information concerning the other party obtained as a result of an executed contract.
Any proprietary information removed from the State's site by the Contractor in the course of
providing services under RFPBid will be accorded at least the same precautions as are employed
by the Contractor for similar information in the course of its own business.

PROJECT DESCRIPTION
Weld County Department of Social Services is requesting Core service funding to pay providers to
supply parent visitation services in court-ordered cases. This bid will allow the Department to access
Core funding to pay identified providers to provide this service. Presently, the Department has two
case-aides whose primary job is to provide transportation and to supervise visits between children in
foster care and their parents or potential caretakers. They are not adequate to cover the demands of
the caseload. Other Core providers of visitation services have been unwilling to provide a
transportation component, which has limited their effectiveness and utility to the Department. The
Generic Visitation bid will allow the Department to contract with providers to transport children to
visits, supervise the interaction between the children and their parents, return the children to their
placements, and document their observations. Each prospective provider will undergo training
provided by the Department. Providers will be expected to provide safe transportation for the children
and comply with the laws governing safety restraints. They will have current and appropriate auto
insurance, including adequate liability insurance. They will be attentive and nurturing to the children
when transporting them, and will help the children prepare for their visit. The providers will be
expected to monitor the visitation, assuring the children's safety, and that the parents follow the
behavioral criteria determined for the visit. The providers will be expected to use their discretion to
intervene in any visit where interactions are unsafe or detrimental, and they will have the authority to
cancel visits when behavioral parameters have been exceeded. Providers will be expected to give
parents' feedback and encouragement, and reinforce effective behavior. While transporting the
children, providers will be sensitive to any disclosures or other significant protective information
shared by the children. Providers will also be expected to make clear scheduling arrangements with
foster parents and parents over future visitations. Providers will document their observations and send
them to the assigned caseworker.
II. TARGET/ELIGIBILITY POPULATIONS
The capacity of this program will be determined by how many specific individuals will be available
to provide the service. Our case-aides can manage approximately four to five visits per day per
worker. Many families have multiple visits scheduled every week, so case-aides manage
approximately seven to ten families per month. The total number of clients to be served by Generic
Visitation would be seven to ten families, or approximately 30 people per month per worker. If more
than one worker were contracted through this account, the capacity would increase proportionately
with each additional full-time worker. Children ages birth through 18 years old would be eligible for
transportation. Bilingual and Bicultural services will be available to approximately 50 percent of the
clients, as the Department will strive to find a provider who is bilingual and bicultural. South County
will be a primary focus of this program, as transportation and case-aide availability has always been
an issue in South County, so the sub-total of clients in South County to receive this service will be at
least 50 percent. The time-frame needed to complete this service is contingent on court orders, as
some families only require supervised visits for a short period of time, and others need the service for

many months. An average length of service will be approximately three months. The hours per week
needed for the service is also driven by individual court orders, with three to six hours per week the
usual orders.
III. TYPE OF SERVICES TO BE PROVIDED
A. Transportation: Providers will be expected to provide safe and reliable transportation from
foster homes to the Department (or other designated site) and back to the foster home.
Providers will be on time according to a prearranged schedule, and will make clear
arrangements with foster parents for future visits. Children being transported will be in the
proper safety restraints, as prescribed by law. Providers will be attentive to the children's
needs during transport time, and help them to understand what is occurring and to prepare for
visits. They will also be sensitive to helping the children process the visits on the return to the
foster home. Providers will keep foster parents,parents, and caseworkers apprised of any
clinically significant communications made by the children.
Supervision: Providers will monitor the interactions of the parents or potential caretakers with
the children. Safety is the first consideration, and the worker will try to facilitate a safe and
nurturing environment for the children in the visit. They will be authorized to intervene in any
visit where the interaction is detrimental to the children, or to help the family refocus their
behavior. Providers can reinforce positive behavior and model parenting techniques for the
parents. Providers will be authorized to cancel or terminate visits if a parent appears
intoxicated, or under the influence of any substance, or if the parents arrive too late. Visits
will also be cancelled if parents insist on persisting in a behavior that they have been
instructed to discontinue.
B. Documentation: Providers will make a written record of the visits they supervise, to be
passed on to the assigned caseworker where the record will become a part of the case file. The
written documentation will include the facts of the visit(date, who attended, etc.) and the
impressions of the supervisor. They will document the affect of the children and the parents,
the type and level of interaction between the family, and any other pertinent observations. If
there are any critical incidents in the visit, they will immediately notify the caseworker or the
caseworker's supervisor for instruction on how to proceed. Providers may be expected to
videotape visits, and they may be called in to court to testify about what they have witnessed.
IV. MEASURABLE OUTCOMES
A. Transportation: Effectiveness will be determined by the provider's adherence to the schedule.
Providers will be expected to pick up the children at the designated time, have them to the
visitation site on time, and return them promptly to the foster homes after the visit. The
Department will track the efficacy of the transportation component by the provider's
adherence to the schedule.

B. Supervision: The effectiveness of the supervision component will be tracked by three
measures. One measure will be successful reunification. Cases with court-ordered supervision
would nearly always have the primary goal of reunification with family members. Supervised
visitation, by enhancing parental competency will ead to children in care returning to their
homes. Another measure will be those cases that can go from supervised visitation to
unsupervised visitation. Providers will serve the function of screening and observing
interactions and helping the caseworker decide when it is safe to go to unsupervised visitation.
The final measure will be the risk assessment. The risk assessment is done early in the case,
and at various points throughout. By comparing risk assessments on a pre-service and post-
service basis, the effectiveness of the visitation program can be discerned when considered
with the other treatment interventions.
C. Documentation: Providers will be accountable for written communications about their visits.
Caseworkers and the program supervisor will examine the documentation for accuracy,
timeliness, and content.
V. SERVICE OBJECTIVES
The primary need for Generic Visitation is to help the Depaitinent manage an increasing number of
court orders to provide supervised visitation. Maintaining parent-child relationships while children
have been placed in foster care can be difficult and time consuming. Supervised visitation or
"parenting time" as it is referred to by the court is a vital element in treatment planning for families.
Supervised parenting allows the Department to observe interaction between the parents and their
children, and to assess safety needs and the level of bonding. This is vital to treatment planning, as
we need to discern what are the primary problems facing a family that has resulted in our
involvement. For families undergoing the upheaval of court involvement and other life stress,
visitation allows them to see their children. For children in care, it can decrease their anxiety about
their parents, and allow them to address issues vital to their safe return home. We have been unable to
meet the demand with two case-aides and other core providers, particularly when the visits require
transportation. With the Generic Visitation Program, the Department hopes to meet the following
service objectives:
A. Manage an increased caseload effectively;
B. Provide increased time availability for families to have visits;
C. Have the opportunity for families to have more visits in a week;
D. Decrease the amount of caseworker time spent on transportation and visits;
E. Allow more time for the case-aides to focus on specific cases;
F. Decrease the pressure on the case-aides to fit in visits when their schedules are already
full;
G. Allow for best-care standards,by giving the professionals involved in visitation more
time and energy to devote to their clients.
These objectives will be monitored qualitatively by the case-aides, the caseworkers, and supervisors.

VI. WORKLOAD STANDARDS
Workload standards for each worker will be seven to ten families per month, based on a 40-hour
workweek. Providers will serve fewer families proportionate to fewer hours. The Department could
accommodate a full-time provider (40 hours), or a combination of full-time and part-time up to 60
hours a week. The modality of treatment will be transportation, supervised visitation,parenting
education, and documentation. Insurance documentation will be mandatory.
VII. STAFF QUALIFICATIONS
Providers will have at minimum a high school diploma and at least two years of documented
experience in the human service field. They will be subject to supervision by the Department's case-
aides, caseworkers, and supervisors. Current Automobile insurance will be mandatory. Providers
must possess a valid Colorado Drivers License and Automobile Liability Insurance.

separate funding source. Providers will be expected to obtain client
verification from the foster parents when they pick up the children.
This form is to be used to provide detailed explanation of the hourly rate your
organization will charge the Core Services Program for the services offered in
this Request for Proposal. This rate may only be used to bill the Weld County
Department of Social Services for direct, face-to-face services provided to
clients referred for these services by the Department. Requests for payment based
on units of service such as telephone calls, no shows, travel time, mileage
reimbursement, preparation, documentation, and other costs not involving direct
face-to-face services will not be honored. Likewise, billings must be for hours
of direct service to the client, regardless of the number of staff involved in
providing those services. Therefore, it is imperative that this rate be
sufficient to cover all costs associated with this client, regardless of the
number of staff involved in providing these services .
