RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT FOR IN-
HOME RESPITE SERVICES AND AUTHORIZE CHAIR TO SIGN - RVNA HOME
HEALTH CARE, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Purchase of
Services Agreement for In-Home Respite Services between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, and RVNA Home Health Care, Inc., commencing
September 1, 2001, and ending December 31, 2001, with further terms and conditions being as
stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the Purchase of Services Agreement for In-
Home Respite Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and RVNA Home Health Care, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 1st day of October, A.D., 2001, nunc pro tunc September 1, 2001.

PURCHASE OF SERVICE AGREEMENT AMENDMENT
THIS AGREEMENT,made and entered into this 1"day of September,2001 by and between the County of Weld,State
of Colorado, by and through the Board of County Commissioners of Weld county, on behalf of the Weld County
Division of Human Services'Area Agency on Aging,hereinafter referred to as"AAA"and RVNA Home Health Care,
Inc.hereinafter referred to as"RVNA."
WITNESSETH:
WHEREAS,"AAA"desires to purchase home health care for Weld county low-income elders from"RVNA,"
WHEREAS,the"AAA"and"RVNA"are desirous of amending the original agreement of January 2,2001.
NOW THEREFORE,it is mutually agreed:
1. The"AAA"will reimburse"RVNA"a total of$32,175 for 2715 units of home health care for 25
people.
2. The"AAA"agrees to reimburse"RVNA"$11.85 per unit of services for general in-home services.
3. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated
in this Amendment.
4. The terms of this Agreement extend from September 1,2001 through December 31,2001.
5. This Agreement may be terminated up thirty(30)days written notice by either party. This Agreement
shall be terminated immediately in the event of loss of funding to the"AAA".
IN WITNESS WHEREOF,the parties hereto have executed this Agreement on the day first written above.

SUBJECT: Amendment to the Purchase of Services
Agreement between the Weld County Area
Agency on Aging and RVNA
Enclosed for Board approval is an amendment to the Purchase of Services Agreement
between the Area Agency on Aging and RVNA. The original contract for services was
$21, 175 for 1787 units of in-home services. The amendment increases the agreement
to $32,175 for 2715 units of in-home services.
