RE: APPROVE COBRA POLICY AMENDMENT TO HEALTH INSURANCE PLAN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been requested to amend the COBRA Policy portion of the
Health Insurance Plan, stating Weld County Government will no longer offer the Dental and
Vision Assistance Plans to terminating employees electing to take the COBRA coverage for
Health Insurance, and
WHEREAS, effective June 4, 2001, the COBRA election will only include the Health
Insurance currently being offered by Weld County Government, and
WHEREAS, after review, the Board deems it advisable to approve the above stated
amendment.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the amendment to the Health Insurance Plan stating that, effective
June 4, 2001, Weld County Government will no longer offer the Dental and Vision Assistance
Plans to terminating employees electing to take the COBRA coverage for Health Insurance,
and the COBRA election will only include the Health Insurance currently being offered by Weld
County Government, be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 4th day of June, A.D., 2001.
