RE: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT AND
AUTHORIZE CHAIR TO SIGN - MEALS ON WHEELS, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, and
Meals on Wheels, Inc., with terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement Amendment between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and Meals on Wheels, Inc., be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 4th day of June, A.D., 2001.

SUBJECT: Amendment to the Purchase of Services
Agreement between Weld County Area Agency
on Aging and Meals on Wheels
Enclosed for Board approval is an amendment to the
Purchase of Services Agreement between the Area Agency
on Aging and Meals on Wheels. The original agreement for
services was $31,000 for 8,708 meals for 117 clients. The
amendment increases the agreement to $37,952 for 10,660
meals to be served to at least 117 clients.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1 day of May, 2001 by and between the Weld
County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA," and
Meals on Wheels, Inc., hereinafter referred to as "MOW."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home delivered meals from "MOW", and
WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of
January 2, 2001,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse " MOW" $37,952 for 10,660 home delivered meals for 117 persons,
with the goal of reaching 48 rural and 18 Hispanic elderly.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise
stated in this Amendment.
3. The terms of this Amendment extend from May 1, 2001 through December 31, 2001. This
Amendment will be terminated immediately in the event of loss of funding.

above.
