RE: APPROVE CONTRACT CHANGE ORDER LETTER FOR STATE PER CAPITA
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Change Order Letter for the
State Per Capita Contract between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2001, and ending June 30, 2002, with further terms and
conditions being as stated in said change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract Change Order Letter for the State Per Capita
Contract between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 21st day of May, A.D., 2001.

This Contract Change Order Letter is authorized under paragraph 9 of the Contract with contract routing number PO FAA
OLL9807021 and contract encumbrance number PO FAA OLL9807021. That Contract has been amended by Contract
Renewal Letter 1,contract routing number PO FAA OLL9900578,Contract Renewal Letter 2,contract routing number PO
FAA OLL0000730,Contract Renewal Letter 3,contract routing number PO FAA OLL0100748. The Contract,as
amended,is hereinafter referred to as the "Original Contract". A copy of the Original Contract is incorporated herein by this
reference,made a part hereof and attached hereto. The Original Contract is between the State of Colorado,DEPARTMENT
OF PUBLIC HEALTH AND ENVIRONMENT and WELD COUNTY DEPARTMENT OF PUBLIC HEALTH&
ENVIRONM. This Change Order Letter covers the term from July 1,2001,through and including June 30,2002. The
parties agree that the maximum amount payable by the State for the eligible services referenced in paragraphs 1-6,inclusive,
of the Original Contract is increased by TWO HUNDRED FIFTY-THREE THOUSAND TWO HUNDRED FIFTY-
SEVEN DOLLARS,($253,257.00) for a revised total financial obligation of the State of ONE MILLION FIFTY-THREE
THOUSAND FOUR HUNDRED THIRTY-FIVE DOLLARS,($1,053,435.00). The budget,which is attached hereto as
"Attachment 1",is incorporated herein by this reference and made a part hereof The fast sentence in paragraph 7 of the
Original Contract is hereby modified accordingly. All other terms and conditions of the Original Contract are hereby
reaffirmed. This amendment to the Original Contract is intended to be effective as of July 1,2001. However,in accordance
with section 24-30-202,C.R.S.,as amended, in no event shall this amendment be deemed valid until it shall have been
approved by the State Controller or such assistant as he may designate.
Please sign,date,and return all 4 originals of this Contract Change Order Letter as soon as possible,to the attention of:

SUBJECT: State Per Capita Contract Change Order
Letter
Enclosed for Board review and approval is Contract Change Order Letter Number 1 to the Per
Capita contract between the Colorado Department of Public Health and Environment and Weld
County.
This change order letter will provide funding to Weld County for local public health efforts in the
amount of $253,257 for the time period July 1, 2001 through June 30, 2002. This is an increase
of$35,221 over the current year's agreement.
I recommend your approval of this change order letter.
